Resumen:
Los carcinomas biliares son tumores malignos infrecuentes, pero no excepcionales, cuyo pronós-
tico ha sido tradicionalmente ominoso a corto plazo por un doble motivo: su carácter clínicamente si-
lencioso hasta fases avanzadas de invasión tumoral y su extensa invasión local en áreas de difícil
abordaje quirúrgico, como el hilio hepático y la zona pancreaticoduodenal. La obstrucción al flujo bi-
liar es la consecuencia principal de su desarrollo en estas localizaciones anatómicas, y su resolución
con frecuencia prolonga la vida aun en ausencia de resección tumoral. En los últimos años se ha
producido un notable progreso en las técnicas de radiología intervencionista destinadas a paliar la
obstrucción biliar mediante colocación de catéteres transbiliares percutáneos o por vía endoscópica;
el drenaje biliar externo o interno conseguido de esta forma mejorar con frecuencia la supervivencia y
evita la intervención quirúrgica, que sólo es curativa en casos excepcionales y está gravada con una
importante morbilidad y mortalidad en la ictericia tumoral. El 80% de los pacientes con carcinoma de
vesícula tienen Colélitiasis, lo cual sugiere una relación etiológica entre la presencia de cálculos y la
posterior degeneración maligna del epitelio de la vesícula biliar. Su prevalencía en material de autop-
sia es del 0,5%; se encuentra 4 veces más en mujeres que en varones, en concordancia con la fre-
cuencia relativa de Colélitiasis en ambos sexos. Su prevalencía es también mayor en los grupos de
población con elevada incidencia de Colélitiasis, como las tribus indias del sudoeste norteamericano
y los chilenos. Para el paciente con Colélitiasis, sin embargo, el riesgo de degeneración maligna de
la vesícula es pequeño, ya que su incidencia es sólo del 1%. Por razones desconocidas, este riesgo
es sustancialmente mayor en pacientes con vesícula calcificada (vesícula de porcelana), por lo que
en este grupo está indicada la colecistectomía profiláctica. La edad media de presentación clínica
oscila alrededor de los 70 años.

Summary:
The biliary carcinomas are malignant tumors infrequent, but nonexceptional, whose prognosis has
been traditionally ominous short term by a double reason: its character clinically quiet until phases

ternal or internal biliary drainage of this form to improve frequently the survival and avoids the opera-
tion, that is curative in exceptional cases and is only taxed with an important morbidity and mortality in
tumorlike jaundice. 80% of the patients with vesicle carcinoma have Colélitiasis, which suggests a
etiológica relation between the presence of calculations and the later malignant degeneration of epite-
lio of the biliary vesicle. Su prevalencía in autopsy material is of 0.5%; one is 4 times more in women
