AFFIDAVIT

I, ANTONIO T. VALENCIA, of legal age, married and with residence and postal
address at Phase 1 San Isidro Village City of San Fernando (P) after having been
duly sworn to in accordance with law, depose and sayeth;

1. That I am a permanent employee of the Lands Management Service, DENR,

Region III, Diosdado Macapagal Regional Government Center, Maimpis City of
San Fernando (P).

2. That presently I am assigned in the Original and Other Surveys Section of the
Surveys Division as Carto 1 Survey Plan Checker.

3. That as a Survey Plan Checker it is my duty to check the correctness of

details on survey plan against the survey returns.

4. That on June 24, 2009, I have signed and checked portion of certain plan Csd-
03-018982 upon performing my duty thereon.

5. That recently after more than one and half years said plan (unapproved)
became the subject of controversy or claims and conflict whereby I and other
co-employees of our department through whom the plan passed were called
upon for some clarification or comment.

6. That on February 22, 2011 I and my unit personnel were called by the Chief,
Regional Surveys Division and showed us the lot data computations of plan
Csd-03-018982 and we noticed that there were some additions in the
claimant designation portion of Lots 3631-G wherein Irrigation Canal was
added; Lot 3631-I, wherein Irrigation Ditch was added and Lot 3631-J,
wherein CEMETERY was added. Also on all lots the initials AGL was added on
the “Geodetic Engineer” portion. All addition was visually of different
penmanship with that who prepared the lot data computations.

7. That, as far as I’m concerned, I just did my official technical duty over said
plan having no knowledge as to present issue on claims and conflict on the
lot subject of unapproved plan Csd-03-018982.
