AFFIDAVIT
I, KENNETH C. SUCGANG, 20 years of age, single, Filipino and a resident of Brgy. Saludares, Dingras, Ilocos Norte, after having duly sworn to in accordance with law do hereby depose and state the following facts, to wit: 1. I know LARRY MANIBOG, JR because we are friends since childhood; 2. As a friend, I used to visit their house at Brgy. Root, Dingras, Ilocos Norte; 3. On about 4:00 oclock in the afternoon of November 19, 2010, I went to the house of my friend LARRY MANIBOG, JR since we have agreed to buy a bicycle part at Laoag City, Ilocos Norte using the wave of LARRY MANIBOG, JR; 4. After buying the said bicycle part, we immediately went home riding the same motorcycle; 5. While traversing the national highway of Banna-Nueva Era road within the territorial jurisdiction of Brgy. Dancel, Dingras, Ilocos Norte on our way home and considering that it was already dark, a driver could not exactly saw any approaching vehicle without a headlight or any warning lights as the LTO may imposed particularly to the approaching any vehicle; 6. The said national highway has no street lights, thereby making the street so dark during the night; 7. The photographs of the scene of the accident taken by the police investigators of PNP-Dingras, Ilocos Norte is hereto attached as Annex A hereof ;

8. While on our way, a fast approaching KURONG KUROG without a headlight or warning lights comes from the opposite direction, them negligently and recklessly bumped the motorcycle where we are riding causing as to fall from behind several meters from the motorcycle; 9. As a consequence of the accident, we were rushed to Dingras, District Hospital, my medical certificate issued by DR. CATHERINE G. DAMASO under Record No. 05-96-98 dated 7 April 2011 is hereto attached as Annex B hereof. I HEREBY ATTEST to the truthfulness of the foregoing facts.
