This will help in stimulating the patient not to cry. 3. This is to avoid patient from falling. 4. This is allowing her not to focus on you so that she will not cry. 5. For patient to be able to sleep well

Sleep pattern disturbance Independent: related to anxiety 1. Establish rapport to patient by talking to her or playing with her. 2. Allow significant other to participate in the procedure to be done. 3. Raise side rails of the bed 4. Provide patient something to divert her attention with. 5. Provide a comfortable environment. Risk for aspiration related Independent: to increase mucous 1. Position patient in secretion side lying position. 2. Position patient in high back rest if feeding 3. Provide chest tapping if patient coughs. 4. Do not force patient to drink or sip water if she is crying.

1. This will allow the secretion to come out. 2. This will help avoid the patient from being aspirated. 3. To help loosen patients secretion thus providing an effective airway passage 4. forcing the child to drink will increases tendency of aspiration.

Dependent: 5. Suction as ordered 5. help in removing secretions more effectively.
