AND CREW RELEASE FORM

This Letter of Agreement is a legal binding agreement for Chasing Shadows (hereinafter Production). This agreement is made and entered into, effective the date of my signing, by and between Joseph Eulo located at 120 W. Third Ave, Apt 11, Roselle NJ (hereinafter Director) and I: _________________________________ (hereinafter Actor or Crew).
This Letter of Agreement will cover the employment of Actor or Crew Member by Director in this Production. The terms and services are listed below. All Actors and Crew will be on time for all calls, production meetings, table reads and stay through to the end of their principal shooting. They will be released by the Producer and/or Producers representative whom will be made known to all Actors upon arrival on location by the Producer. Actors and Crew will provide their own transportation. Actors will also provide their own wardrobe at the approval of the director. There will be a secure place for all Actors and Crew to change into wardrobe, use the lavatories and relax during waiting time. Compensation: All actors and crew will be furnished with food & beverage on shooting dates, credit (listed in order of appearance / rank per Directors right) a link to and permission to use production photographs, copy of the final project, as a HD H.264 .mov file, DVD, and link. Credit appearance will be subject to the Directors right. There will be no other payment stipulated other than what is specified above. The Director shall own all rights in every kind in the results and proceeds of Actors services hereunder. Director shall have the unlimited right throughout the universe and in perpetuity to exhibit the in all media, now or hereafter known. All Actors and Crew services are a work for hire. I understand that the Productions title listed above is tentative and can change without releasing me from the terms of this contract. I confirm that any and all material furnished by me for the Production is either my own or otherwise authorized for such use without obligation to me or any third party. I grant the Director the irrevocable and unrestricted right of use of my name, likeness, image(s), voice, and biographical material produced via my participation in the Production. Director may exhibit, advertise, promote, and otherwise exploit Production or any portion thereof in any medium, whether or not such uses contain audio and/or the visual reproduction of myself and whether I am identifiable or unidentifiable. I understand that the Director has the right to use the materials created for the Production in any way he chooses and I have no right to inspect or approve those materials. I further agree that my participation in the Production confers upon me no rights of use, ownership, or copyright. I understand that all materials and intellectual properties produced in association with my participation become properties of the Director. I release the Director, his employees, individuals assisting with the Production, agents, assigns, and/or third parties associated with filming locations from all liability which may arise from any and/or all claims by me or any third party in connection with my participation in the Production. I agree to pay for damages to any and all items, property, and/or equipment related to the Production that results from my negligent and/or reckless behavior. It is understood that the Director is under no obligation to screen, broadcast or distribute the Production. I give the right to the Director to assign all terms stated in this contract. I also understand that by agreeing to the terms of this contract, I am not guaranteed participation in this Production.
