Ineffective airway clearance This is the Top Prioritized nursing problem because this is considered as part of Physiologic need,

according to Maslows hierarchy of needs, physiologic need is should be one of the most prioritized, under Physiologic needs, we could consider this as part of the oxygen since airway clearance the oxygen taken in by the patient and according to OFFTERAS, Oxygen is should be most prioritized. (Also, According to ABC, Airway should be the prioritized problem since the main problem we are dealing with is not a cardiac in Origin.) Decreased cardiac output / Ineffective tissue perfusion This is second in the prioritized problems. Although this is also part of Physiologic need in which it is the same with ineffective airway clearance. This can be considered as fluid in OFFTERAS which is only second to Oxygen, I considered this as Fluid since these nursing problems are talking about blood. (Another reason is According to ABC, circulation is third and cannot be considered as the most prioritized problem since Airway should be more Prioritized. The reason why these two nursing Problems Are clustered is that This can be solved simultaneously, If the Decreased Cardiac Output would be Solved, so is the ineffective tissue Perfusion) Acute pain This is the Third nursing Problem Identified. This is because although this could be part of Physiologic need according to Maslows Hierarchy And according to OFFTERAS, Rest is 6th in the Ranking which is lower than Oxygen and Fluid. I included acute pain as Rest since this affects the quality and time of the patients rest periods, With the presence of acute pain, rest is disturbed. Activity intolerance / Fatigue These are the Fourth Nursing Problem. This is part of the Physiologic need according to Maslows hierarchy of needs, according to OFFTERRAS, activity is 7th in the ranking in which these two nursing problems belong to. I considered These two as part of Activity and clustered it together since Fatigue and Activity Intolerance go hand in hand together, If there is fatigue activity intolerance follows. Risk for constipation This is the Fifth nursing Problem. This is Considered part of Physiologic need according to Maslows hierarchy of needs, According to OFFTERRAS, elimination is 5th in the ranking which is higher than that of Rest and Activity, I put it as the Fifth Nursing Problem because it is a Risk Diagnosis, In comparison to the First Four nursing problems, this last two nursing problems are just Risk Diagnosis. And active Problems should be taken priority more than this problems Risk for injury

This is the last Nursing Problem. This is part of Safety and Security according to Maslows hierarchy of needs, which is only second to Physiologic needs where all the other Nursing problems belong to.
