Chapter VIII EVALUATION AND SUMMARY The nursing assessment described in this study is a deliberate, systematic process of gathering

and analyzing data to identify and continuously validate health and nursing problems in a family. The assessment process offers the student nurse the opportunity to strengthen the skills in making sound judgment using data based on evidence. It challenges the student nurses mind in evaluating assumptions or premises and arrived at valid conclusions. By going through the process of data collection and analysis, the student nurse learned that clients have varied views in life, that they hold different aspirations and those they respond to situations or problems in unique ways. The Family P has existing actual and potential health problems. Some of those are already identified by the family but then, actions are not taken because they dont give much attention to it or that they are unaware of the possible harm that it can give them. During interviews, health teachings regarding unhealthy practices were given, at the same time, in order to educate the family. They identified poor environmental condition and improper sanitation as their main health concern. Furthermore, they are the ones who enumerated possible solution to the identified main family problem. The student nurse only served as a guide in formulating solutions to the problem while the family members are the ones who performs the implementation of different identified

solutions. The family promised to put into actions whatever they had planned in order to solve their problems on sanitation and identified themselves as the primary benefactors. Recommendations With the identified family nursing problems shown at previous chapters, the student nurse recommended the following activities/alternatives: 1. Make a house to house visit to assess and update the familys health and stress the importance of personal hygiene and proper environmental sanitation through health teachings and motivation as to encourage the family to take proper action. 2. Teach and reinforce the family in promoting and maintaining its waste and garbage management to facilitate cleanliness, orderliness, and the segregation of biodegradable and non-biodegradable. 3. Continue to encourage each member of the family to prepare nutritious foods for the family to help boost their resistance against acquiring any diseases. 4. Teach and emphasize the possible effects of the different personality disturbances practiced by the family which could somehow affect way of life and health. 5. Continue to educate the family on proper health principles and practices to increase their knowledge and awareness in making decisions with respect to taking appropriate health action.
