We are excited to announce a trip opportunity for the Concert Choir, VI, Serenade, Sweethearts,
and Mens groups! These groups have been invited to perform with the Heritage Festival in Anaheim,
California. We are excited to have the opportunity to participate in a festival of this nature. This
festival is run at Fullerton College from April 3-5 , 2009. The festival highlights include:
Because of the nature of traveling with these performing groups, it is important that all
participating students are involved for the sake of the ensemble. As a result, please discuss the options
with your student and return the attached permission slip by Friday, December 19th 2008.
Payment schedule and a preliminary itinerary are attached. As the trip approaches we will
have a more detailed itinerary that will go out to your student. Please note that your student will be
missing a full day of school. It is his/her responsibility to make prior arrangements for any and all
make-up work for missed classes.

Payment Schedule:
Late Fee
An additional $20 late fee will be applied to any monthly payment not received by the posted
due date.
Participation on this trip is based on students maintaining passing grades in all of their
classes. As the students will miss one full day of school, it is up to your student to make
proper arrangements for make-up work. STUDENTS CAN BE REMOVED FROM
THIS TRIP BY THE ADMINISTRATION AND/OR TEACHER IF THEY DO NOT
MAINTAIN PASSING GRADES OR IF THE STUDENT’S BEHAVIOR INDICATES
THEY COULD BE A DISTRUPTION ON THE TRIP. STUDENTS WILL NOT
RECEIVE A FULL REFUND IF THEY ARE REMOVED FOR EITHER
DISCIPLINARY OR ACADEMIC PROBLEMS.

I have read the information regarding this trip to Anaheim for the students in the Palo Verde
High School Choir. I understand and agree to abide by the payment schedule and cancellation
policy. I also understand that if my son or daughter is removed from this trip due to
academic or disciplinary ineligibility, the same cancellation policy will apply. My
nonrefundable deposit of $50 accompanies this consent form or has already been deposited.
