INTRODUCTION
Sir J. J. Group of Hospitals, Mumbai is one of the premier medical institutions of India. It ranks among the ten best medical colleges of our country. In 1834, Sir Robert Grant was appointed the Governor of Bombay and he directed his attention to the expediency of establishing a systematic and scientific institution in Bombay city for imparting medical knowledge to the natives of this province. Consequently, this institution was started as a medical school at the Esplanade in Fort area of Bombay in 1835, making it the oldest in India and probably the first on this side of the Nile , first of its kind, for imparting medical knowledge to the natives of this country. In March 1838, Sir Jamshetjee Jeejibhoy donated Rs. One lakh for building new native general hospital at present site. The shifting to the present site and renaming it to Sir Jamshetjee Jeejibhoy Hospital and Grant Medical College occurred in May 1845. Since then it has veritably become a part of the heritage of Mumbai. Its history is an integral part of the History of Medicine in India. It has been in fore front in medical re lief during natural disasters, epidemics, riots, bomb blasts, accidents and other calamities, not only in Maharashtra but also in other parts of our country. The medical relief teams of the institute were first to rush at the time of earthquakes in Latur (1992) and Kutch (2002) and as far as Nicobar Island during Tsunami disaster in 2005. During the 2005 deluge in Mumbai, the institution had done commendable work during Leptospirosis and fever epidemics. Health relief also was carried out during the mass food poisoning (2005) and outbreak of gastroenteritis in 2006.

LOCATION
Greater Mumbai Metropolitan area or Brihin Mumbai Municipal Corporation (BMC) area is divided in two revenue districts viz Mumbai city District and Mumbai suburban District. Greater Mumbai is entirely urban. Sir J. J. Group of Hospitals is situated in the southern-central part of Greater Mumbai. This is the most populated and congested part of the city. The area around Sir J.J.Group of Hospitals is mainly thickly populated residential area with some industrial zones, very clo se to the Mumbai docks and three major railway tracks, namely Central, Harbor and western railways. There are many old residential buildings around this area and during monsoon there is danger of house collapse. Due to heavy traffic and flyover bridges in vicinity, many road accident cases come to us. On the eastern waterfront of the city, much large storage of petroleum products and many ships carrying hazardous consignments may pose threat of gas and chemical leakage. Mumbai being natural harbor, there is threat of Cyclone along the seacoast. Fortunately there are no major cyclones in past in our region. Last one was in 1982 with out any damage.

NEED FOR A DISASTER MANAGEMENT PLAN


A disaster is any event causing widespread destruction and distress. Disaster Management is not a new concept in our institution. In past we have managed man made as well as natural disasters with great success. But management of these of emergencies in past were essentially spontaneous acts without any prior organization. However, the recent rise in the incidence of disasters has alerted us regarding the need for a preplanned disaster management plan. This plan will aim at providing effective and timely relief in times of disaster through organized collaboration of various departments and better use of available resources in a planned manner.

TYPES OF DISASTERS
Natural Calamities a. Droughts, Floods, Cyclones, Earthquake, Land slides, Tsunami etc. Man made disasters b. Rail, road, air accidents; Gas, chemical leakage, fires , c. Riots, Epidemics, Mass food poisoning etc.

PRE-DISASTER PLANNING
Pre-disaster planning consists of: 1. Disaster mitigation & 2. Disaster preparedness

Disaster mitigation
Disaster mitigation focuses on the lessening the hazard that causes the disaster and tries to eliminate or drastically reduce its direct effects before it strike. Examples include strengthening buildings to make them cyclone or 7

earthquake resistant. This part of management is usually done by other government and corporation agencies. However we have to do mitigation for our internal emergencies arising with in hospital campus, interrupting hospital services, threatening lives of staff, patients and visitors and some times necessitating mass evacuation. Fire; building collapse, bomb threats, flooding, an explosion, drinking water contamination, food poisoning etc can cause internal disasters.

Disaster preparedness
Disaster preparedness focuses on plans to respond to a disaster threat or occurrence. It takes into account an estimation of emergency needs and identifies the resources to meet these needs. It also involves preparation of well-designed plans to structure the entire post-disaster response and administrative preparedness by way of stocking of supplies, developing planes for rescue and relief.

RISK ASSESSMENT AND VULNERABILITY ANALYSIS


1) Drought: Due to scanty rain fall the underground water level goes down drastically causing acute shortage of water, leading to scarcity of water for human and animal consumption and crop failure. Drought is not a problem in our part of the state as we get our water supply from municipal water supply, coming from reservoirs 2) Floods: The only source of flooding in our area is from heavy rains especially if it coinciding with high tides. The low-lying areas get flooded and this may trigger house collapses, landslides and epidemics of water borne diseases. This may also cause problem of rail, road and air transport and may lead to accidents. 3) Cyclones: Being an island city, the costal areas are prone to gusty winds and cyclonic impacts. Fortunately, in the recent past, there have been no major cyclones in our region. The last cyclone in Mumbai occurred in 1982 and it did not cause any damage. 4) Earthquake: Though Mumbai city comes under the earthquake prone belt, we have not experienced major quakes in recent past. Last major earth quake occurred in 1951, measuring VIII on the scale. During earthquake there is risk of large scale house collapse, collapse of flyover bridges, bursting of dames, reservoirs and major water supplying huge pipes, causing flooding. 5) Tsunami effect: This is a new addition to the list of disasters. This has never occurred before in this part of world. Last year it caused major casualties all over the world. It is associated with an under water earthquake which causes a large mass of sea water to displace. The

resultant enormous tidal waves are then responsible for massive destruction and large number of casualties on the shores. 6) House collapse: This is more frequent type of disaster in our area where we have large number of timber framed old buildings especially in ward A, B and C. 7) Accidents: Modernization of lifestyle and significant progress in transport and communication sector has unfortunately increased the incidence of rail, road as well as air accidents. We can expect major casualties from rail accidents since our hospital is located in proximity to three main railways, namely Central, Harbor and Western railways. 8) Gas leakage: There are numerous factories in this part of the city, manufacturing and storing hazardous gases and chemicals. Leakage of obnoxious gases from these factories can give rise to large-scale damages. 9) Fire: Greater Mumbai is greatly diversified and practically has every type of fire risk. The fire can arise from following sources. ? Large numbers of closely built timber framed buildings . ? ? High rise buildings with inadequate fire fighting facilities . ? ? Commercial ? activities in Kalbadevi, Bhuleshwar,

Bhendibazar, CP tank etc. ? Large storage tanks of petroleum products along P Dmello ? Rd.

EMERGENCY AND TRAUMA CARE SERVICES Notification of Disaster/ Emergency


? The hospital may be notified of a disaster from various sources like ? o State level Disaster Control room o Directorate Control room o Police o Fire services ? The telephone operator on duty directs the emergency call to Casualty ? Medical Officer on duty. Casualty Medical Officer on duty should identify the person giving the information and then enquire about the nature and magnitude of the emergency, the location, time, possible number of casualties and approximate time of arrival. ? Casualty Medical Officer on duty should immediately inform the RMO ? on duty and senior resident doctors of concerned specialty stationed in emergency ward no. 4, on the ground floor of main hospital building. ? The RMO on duty should cross check with the informing authority and ? try to find out other details regarding the emergency situation and inform Superintendent of the hospital. ? The RMO on duty should station him self at casualty department and ? mobilize transport equipments like wheel chairs, trolleys, call additional CMO if there are large number of victims and help the CMOs in the process of documentation and admission. ? The RMO on duty should also alert other emergency departments like ? Radiology, Clinical Laboratories, Matron, Pharmacist, Supervisor of lift services, Class four supervisor and the Sanitary Inspector so that adequate man power is available like X-ray technicians, lab

technicians, nurses, lift operators, ward attendants and sweepers. The pharmacist ensures adequate supplies. ? The RMO on duty should also inform and immediately call for security ? personals to provide adequate security in the casualty area. If required, the police should be informed as well. ? The RMO on duty should also inform the Senior RMO. ? ? The Senior RMO is the over all coordinating officer in the casualty ? center.

CONTROL ROOM
Control room is essential to coordinate the disaster management activities. It will function from the present RMO duty room situated on the ground floor of the main hospital building, near the casualty. Senior RMO will be in charge of this control room. This room will function round the clock and hotline connection will be provided. This room will be communicating with control rooms at state level as well as with directorate control room. Statistician from Medical record department will help in compiling the data. This will function under direct supervision of hospital superintendent. A media spoke person may be appointed by the Dean.

Functions of control room


? Compilation of data regarding of disaster victims. ? o Full, name, age, sex. o Identification Marks. o Address, date & time of admission, registration no. o Ward No., Treating unit, Nature of injury, diagnosis. ? Dissemination of the data to higher authorities a ? ? Media briefing, press notes ? ? Analysis and application of data ? ? Inter-departmental coordination ? ? Reporting to be done as per pre-determined uniform formats. ?

SIGN BOARDS
Signboards should be prepared and placed at appropriate sites in the hospital campus to direct the flow of patients and vehicles, particularly at the entry points. Signboards need to be prepared to indicate evacuation routes in the hospital for internal emergencies.

ARRIVAL AND TRIAGE


Patients may arrive at the casualty by means of, ambulances and private vehicles. Many patients may already have been classified and stabilized at the emergency site and come with clinical notes and identifications. Triage upon arrival makes it possible to review the severity of the condition. Triage is the process by which victims are sorted according to severity of injury. Immediate attention will have to be paid to the patients who are serious and can be saved. The goal of triage is: ? To select those patients in greatest need of medical attention ? ? To ensure that patients present for treatment only to the appropriate ? forewarned medical specialty as a means of conserving limited personnel and supply resources. The Casualty Medical Officer on duty should do triage primarily and once the patients are transferred to specialty departments, they should be re-triaged by the senior consultants from that specialty. The area of triage upon arrival at the hospital should be the only point of entry for victims of mass casualty. The best triage cla ssification is also the simplest: ? Lightly injured - Green colored flash card or green ribbon ? Lightly injured patients are not disabled and require minimum treatment. ? Seriously injured - Orange colored flash card or orange ribbon ? Seriously injured patients require definitive care but are not urgent need of treatment ? Critically injured - Red colored flash card or red ribbon ? Critically injured patients required rapid, life saving care.

? Brought dead - White colored flash card or white ribbon ? The usage of tria ge colored flash cards or colored ribbons to classify and identify, whatever the model, will be standardized whenever possible.

IDENTIFICATION AND REGISTRATION


The Medical Record Department has design in advance the documents for identification and registration of the patients who are admitted. Forms for recording data, identification cards and cards for triage should be available. Green, Orange, Red and white flash identification and triage classification cards are standardized for this purpose. The combination of the identification and triage cards is a better option. Registration forms are designed in a way that it collects the required information with out much delay in the admission process.

REFFERAL PROCESS
The CMO on duty, after documentation, directs the patients to different specialty departments for treatment as per the need. Adult patients with injuries should be sent to emergency ward No. 4, situated close to casualty, on the ground floor of main hospital building. In this ward, lecturers and resident doctors from the emergency units on call on that day will asses the patients, re-triaged them. On arrival resuscitation measures are carried out and details should be entered in the indoor case papers. Concerned unit heads are informed immediately. Those adult patients who need medical line of treatment should also send to ward 4 and examined by residents and lecturer from medicine department. After examination, if required, they should be sent to medical wards for admission. No patients should be kept for observation in ward 4 on OPD basis.

Pediatric age group patients should be sent to pediatric departments with hospital attendants with due care.

ACCOMODATION
During any disaster, it is likely to get large number of patients in a short interval of time. In order to accommodate them, we have a plan for immediate expansion of indoor facilities at a short notice. ? Expansion of indoor space by utilizing all available space in ward 4 ? and adjacent area like verandah on the both sides of ward 4. In this we can accommodate 30 beds in ward 4. ? A ward has been ear marked for expansion of emergency bed strength ? in such situations where we need more than 30 emergency beds on second floor of main hospital building, adjacent to MICU. In this we can admit 28 more cases. ? Discharge those indoor patients who can be discharge ? ? Transfer some indoor cases, already admitted before, to some other ? wards ? Restrict admission to only critically ill patients ? ? For pediatric cases, 60 on pediatric medicine and 20 on pediatric ? surg ery side can be accommodated. Additional beds space is created in the adjacent Child Welfare Center if need arises.

Accommodation for Staff


Those medical personnel, deputed from outside for medical relief work are be provided accommodation at the old hostel, guest house located in the building No. 5. Those lecturers, who are not having campus accommodation, are to be given accommodation/duty rooms in the 300 residential quarters, BMS quarters and above the CWC. Wardens are informed for this. Matron will make arrangements for accommodation for the nurses coming from outside in the nurses hostels and girls hostel.

EQUIPMENTS AND MEDICAL/SURGICAL STORES


Adequate buffer stock of essential emergency medicines and surgical items like intravenous fluids, dressing materials, splints, suture material, oxygen cylinders, suction apparatus etc is stocked in emergency ward with a preparation of comprehensive list and a regular check is kept on this by duty RMO on daily basis (list attached).

Approximate time of arrival of patients. ? ? Inform RMO on duty/ Senior RMO/Superintendent ? ? Inform the on duty lecturers and residents stationed in ward 4. ? On arrival of patients ? Receive the disaster patients ? ? Registration and identification of the patients ?

? Call additional staff on duty. ? ? Keep adequate blood units in stock ? ? Get additional blood sto ck from other blood banks ? ? Arrange for donation camps with the help of NGOs ?

Forensic department
? Arrangements for carrying out postmortems ? ? Arrange duties of all staff members round the clock ? ? Provide Embalming and cold storage facilities. ?

Contribution of Para clinical and Non-clinical departments


? Depute some staff to clinical departments for additional help. ?

Emergency arrangements for mass Casualties Personnel


? On duty doctors, nurses, technicians, attendants and other staff on ? emergency duty should not leave the place of duty unless relief staff arrives for next duty.

Material
? Emergency medicines and surgical items required for treating 50 ? emergency cases kept in readiness in separate specially labeled storage cupboards in emergency ward. ? Keys of these cupboard should be available with on duty sister or ? staff ? No items from these cupboards should be used for routine cases ? ? These items should be checked periodically for expiry dates ? 25

? Those with expiry in near future should be replaced with fresh stock ? ? List of these should be prepared and available with sister of ward 4 ? ? Sufficient X-ray films should be available with radiology department ?

Space Expansion
? First 30 cases to be accommodated in the emergency ward No.4 ? ? Other cases admitted in ward 4, who are less critical should either ? be discharge or shifted to the wards attached to the treating units ? Additional 30 cases, if come, to be shifted to the vacant ward ? earmarked for this purpose, on second floor of main hospital building, opposite MICU. ? Those patients from regular wards, who can be discharged, should ? discharge to create additional space. ? Well settled, previously admitted patients, should be kept together ? in one ward, irrespective of units, and keep the disaster victims in the remaining vacant wards. ? For further expansion, if need arise, surgical ward No. 18 and ? medicine ward No. 8 can be commissioned.

Initiations
? Emergency control room starts functioning ? ? The list of staff members required for emergency duty with contact ? Nos. is made available in the control room ? All operation theaters are alerted and OT staffs and OT attendants ? are called for duty. ? AC operators of AC plants for OT are instructed for continuous ? working of AC plant

? Emergency staff members, who are on leave, should be call back, ? canceling their leave. ? No leave to be granted during this period unless it is must ? ? Routine OPD services will be maintained with help from staff ? deputed from para and non-clinical departments ? Admission will be restricted to emergency cases ? ? HODs are requested to draw out detailed plan of duties to be ? assigned to staff members to look after casualty services, emergency services, indoor services and ICUs. ? The duty list should be prepared by HOD and a copy to be sent to ? Dean, Superintended, Control room, duty RMO, duty CMO, Telephone operator ? Superintendent should prepare emergency duty list of RMOs and ? CMOs. ? RMOs and Superintended should take regular rounds of casualties ? and emergency ward ? and Dr. M. J. Algotar, Professor of surgery and nodal officer for surgery department in such situations. Unit head will rush to the emergency ward and take the stalk of the situation with the nodal officer. Department head will be informed by Dr. M. J. Algotar. He will also call the additional surgical residents, lecturer and other senior staff members from other units for help, if the need arise, to tackle the initial phase of mass casualty. Measures will be taken to expand the existing indoor bed strength for additional patient load during the disaster as mentioned above. Additional operating suits will be commissioned and emergency operations will be carried out. Two, adjacent surgicalunits will be combined and will work as a single unit during such situations, e.g. 17a and 17b units will work as one unit.

Nodal officer on the campus will immediately intimate the lecturers and resident doctors about the disaster and direct them to attend the cases as and when they reach the hospital. He will himself will assess the situation and inform the HOD about the factual position and obtain further directions. The HOD will reach the hospital as soon as possible and take charge of the situation. The lecturers along with the resident doctors will assess the patients and manage them according to the condition. Team of one lecturer and resident will go to the casualty ward to assist board specialty team and also to identify patients requiring neurosurgical management and shift them to operation theatre/ ward/Critical care unit as situation demands. Second team consisting of one lecturer and resident will handle patients who need observation in neurosurgical ward. They will also look after ventilator management of conservatively treated and post operative patients. Third team of one lecturer and resident will operate as per triage. However if situation demands second team in ward will also operate on patients. The associate professor will supervise and help the teams whenever required under guidance and overall supervision of HOD.

DETAILED DEPARTMENTAL ACTIVATION PLAN TO MANAGE DISASTER The following protocols have been decided upon 1) Natural calamities like earthquakes, landslides, building collapses and deluge On receiving information from the CMO and/or RMO the entire unit shall remain alert in the campus. The nodal officer and a senior resident will remain in the casuality ward. They will do preliminary assessment and emergency treatment. They will be assisted by the medical, paramedical and non medical personnel in the casuality ward. They will also co ordinate with other departments like radiology, blood bank, orthopedics, neurosurgery and vascular surgery departments. In the ward the surgical patients will undergo respective surgeries. The patients who are kept under observations will be monitored Medico-legal work and documentation will be undertaken by the clerk designated with help of police. Active involvement of social worker will be solicited. 2) Man made disasters. Like bomb blast, fire vehicular accidents the nodal officer and a Senior resident will remain in casuality ward to do the preliminary work. All available working hands in the casuality will be will be suitably used. If required additional hands will be requested for. All minor wounds and burns will be treated in the casuality. If observation is warranted they will be admitted to ward. All burns patients should be admitted to burns ward under plastic surgery. They will be seen by the pediatric surgeons, as per hospital schedule i.e. morning and evening. All the patients requiring surgery or observation are admitted to ward by the nodal officer.
