Application Procedures
For Academic Year 2013-2014
FRESHMAN APPLICANTS 1. Fill out the online Undergraduate Application Form. The hardcopy of the application form may be secured from the Office of Admissions and Scholarships or the External Testing Centers. 1.1. Applicants who will take the test in DLSU, Manila must accomplish the online Undergraduate Application Form and submit the Online Application Number. The Office of Admissions and Scholarships will do the printing of the online application form. 1.2. Applicants who will take the test in the External Testing Centers are required to submit a hardcopy or printed copy of the online application form to the contact person of the designated testing center. 2. Proceed to the Office of Admissions and Scholarships or External Testing Center to submit the required documents. Only applications with correct and complete information will be processed. 2.1. Certified Correct Secondary Scholastic Records (Download link provided below) 2.1.1. High school graduates must submit a photocopy of their 4th year high school report card. 2.1.2. International student applicants must submit a photocopy of their secondary school Transcript of Records. 2.2. Two (2) recommendations from any two of the following (Download link provided below): 2.2.1. 3rd or 4th year High School Subject Teachers 2.2.2. 3rd or 4th year Class Adviser 2.2.3 3rd or 4th year Guidance Counselor 2.3. Three (3) identical passport photographs 2.4. A photocopy of National Statistics Office (NSO) Birth Certificate for Filipino Citizens or ACR/ICR/ passport or photocopy of Birth Certificate for non-Filipino Citizens 3. Pay the non-refundable Admission Processing Fee of Php600.00 (Filipino and Dual Citizens) or $50.00 (non-Filipino Citizens or International students) 4. Present the Official Receipt to the Office ofAdmissions and Scholarships and secure the Admissions Kit (includes bag and information materials) and Test Permit. The entrance exam schedule will depend on the earliest testing date available upon the submission of requirements. 5. Take the exam on the scheduled date. All applicants must present the exam permits on the date of examination. 6. Results and status letters can be accessed at the DLSU website in early January 2013. If you cannot access your results,please inquire at the Office of Admissions and Scholarships. TRANSFER APPLICANTS CRITERIA Grade Point Average (GPA) of at least 85% or its equivalent No failure in any subject No incomplete grades No unofficial dropped subjects PROCEDURE 1. Email or submit a complete copy of Transcript of Records (TOR) with previous schools grading system to admissions@dlsu.edu. ph or submit a copy to the Office of Admissions and Scholarships for evaluation. 2. If qualified, fill out the Online Undergraduate Application Form and print a copy. 3. Submit the completed Transferee Application Form with all the required documents: 3.1. Copy of the Transcript of Records (TOR) 3.2. Two (2) recommendations from any two of the following: (Download link provided below) Dean Department Head Professor Guidance Counselor 3.3. Three (3) identical passport photographs 3.4. Photocopy of the National Statistics Office (NSO) Birth Certificate for Filipino citizens or Photocopy of Birth Certificate, ACR/ICR or passport for non-Filipino citizens 3.5. Certificate of Good Moral Character 4. Pay the non-refundable Admission Processing Fee of PhP600.00 (Filipino Citizens) or $50.00 (non-Filipino Citizens). 5. Present the Official Receipt to the Office of Admissions and Scholarships and secure the Admissions Kit and Test Permit. 6. Take the exam on the scheduled date.
