Authentic dated sales invoice/receipt (copy or original) with business identification verifying the purchase of the qualifying product. Product part number must be identified on the sales invoice/receipt as well as the date of purchase and the retail price. B. This form, filled out completely and legibly. Visibly altered documentation will not be honored. C. The original product identification label with UPC Code cut from the product box. 3. Mail the completed form, required proof-of-purchase (dated sales receipt with product part number), and original label w/UPC code to the address indicated below. Product must be purchased between August 1, 2012 and September 30, 2012. All claims must be postmarked by October 31, 2012. Offer excludes purchases of used products or those sold by non-authorized CFMPlus dealers. Claims sent without original label with UPC code will be denied. Offer excludes resellers and distributors of CFMPlus products. Offer NOT valid on products purchased for resale. Products purchased directly from a dealer outside the United States or Canada are ineligible. Please allow six to eight weeks for processing of rebate after receipt of proper documents and UPC label. Rebate check is valid for 90 days from the date of issue. Fraudulent submissions of multiple requests could result in federal prosecution under U.S. Mail Fraud Statures (18, USC, Sections 11341 & 1342.
