This could be: A copy of a current Council Tax statement or Council Tax benefit letter addressed to the Parent named in section 3 A copy of any two of the following documents, addressed to the parent named in section 3: bank statements, utility bills, credit card bills A copy of the tenancy agreement in the name of the parent named in section 3, where the tenancy period must extend beyond the start of the academic year in which the child starts secondary school Evidence of the exchange of contracts confirming the sale or purchase of a new property.

Please send copies and not original documents as we will not be able to return any paperwork submitted. The completed form and supporting evidence must be received by Oasis Academy South Bank by Friday 15th February 2013. However, it is recommended that you submit your application as soon as possible. We will not be able to process your application without all the necessary supporting documents, so please do contact us if you are having problems understanding of finding what you need to send us.
