QUESTIONNAIRE What we eat, how much physical activity we do, how much alcohol we drink or how many cigarettes

we smoke, and also the quality of our social relationships, create the life style on which the health of each of us depends. We are doing a research regarding the life style of the persons over 45 and you belong to the target group which was selected for this purpose. Please read carefully and honestly choose the answer that represents you. The information is confidential and it is used only for processing.
