1. Have your physician fill out the Application/Medical Clearance form. To request a form, please call Seana Hallberg at (603) 740-2574 or email seana.hallberg@wdhospital.com 2. Fax the form to (603) 740-3349 or mail to: Attn:Seana Hallberg, CHaD Family Resource Coordinator 789 Central Ave., Dover, NH 03820 3. Once approved*, pay the $75 registration fee. If family meets required attendance, all funds returned at program completion.
*individuals may be excluded from this program based on complicating medical illnesses or an inability to commit to the length of the program.

Make a lifestyle change and improve overall health!


CLIMB is a 10-week program for teens struggling with weight. Participants and their families will work with the Lifestyle Team to learn the following skills:
Creating balanced meals Proper exercise techniques Identifying emotional traps that lead to overeating How to make good food choices Working as part of a group

CLIMB Mission:
Our goal is to give teens struggling with their weight the tools to achieve a healthy lifestyle. We focus on the family as a unit and provide a multidisciplinary program to obtain a lifetime of better health.

Support for Success


Teens will have access to many forms of support to help them succeed in their weight loss goals. Participants will receive:

Helpful resources such as: cookbooks, fitness ideas and nutritional guideline Peer support Full family membership to The Works Family Health & Fitness Center for the duration of the program Tips for working out at home

Participants:
CLIMB is designed for teens currently in high school grades 8-11 (AT LEAST 13.5) who: Struggle with weight and/ or body image Have physician medical clearance to participate Are committed to an intense program that addresses these issues from multiple angles

Our Lifestyle Team consists of experts in the areas of: Fitness Nutrition Teambuilding and self esteem

Schedule: January 26th- March 30th


10 week program in which nutrition education begins prior to start of program Mon, Wed., & Fridays from 3:15pm5:30pm The Works Family Health and Fitness Center, Somersworth, NH Tuesdays 5pm-7pm. Nutrition education & Support Group (CHaD @ WDH) Recreational activities: Saturdays from 9am-10:30am (location varies)

Cost:
$75* per participant. There is no charge for family involvement. Full amount will be returned to all participants that complete program within guidelines
