Failure of the U/G Cooling water line.

Reason for Inspection: Heavy leak of due to failure in U/G cooling water line, Which lead
the full plant process facility shutdown.

Inspection Summary: Internal inspection was conducted along with Amiantit Specialist and found
that U/G line was totally busted in the Bottom of the TEE-branch connection. Areas around the T-branch connection were excavated and location of the failure was identified. The U/G FRP line was ruptured from the 45 Deg south bottom to bottom sides, fractured area is approximately 750 mm x250 mm. During the excavation for exposing the failure area a lot of foreign rigid materials were observed in the back filled sand. (I.e. Concrete slabs, Metallic re enforcement bars, Scaffolding pipe, Wooden pieces, Gravels-sharp stones see pictures below).Rupture area was very closer to the concrete slabs and the slabs were found in clashed stage with the FRP Pipe external wall.

Inspection Details:

Internal inspection was conducted through the man way and the inspection covered both side, east and west to rupture location.(10 meters away from the man way).6 couplings from the west and 3 from the east were inspected internally and found in good condition and the clearance between the connected spools within the coupling is within the acceptable limit. The internal wall of the pipe was found in good condition and no major observation.

Root Cause of Failure: During the excavation for exposing the failure area a lot of foreign
rigid materials were observed in the back filled sand. (I.e. Concrete slabs, Metallic re enforcement bars, Scaffolding pipe, Wooden pieces, Gravels-sharp stones).Rupture area was very closer to the concrete slabs and the slabs were found in clashed stage with the FRP fitting.

Recommendations : 1. 2. 3. 4. Cut the Damaged portion. Prepare patch piece to be installed in the failure area. Ensure proper surface preparation. Do lamination as per AMIANTIT recommendation.

5. Finally perform barcol hardness test.
