1. Introduction
The curriculum for medical studies is set forth in the German Medical Licensure Act (Ap-probationsordnung fr rzte, AppO). The curriculum and requirements for medical stud-ies are also governed by the study regulations of the Tbingen Faculty of Medical (Studie-nordnung). The licensure act and the study regulations together define the aims and dic-tate the structure, organization and duration of the course of studies, the required medical subjects, the material to be covered in examinations, as well as examination prerequisites and modalities. The AppO is a national policy, whereas the regulations of the medical faculties vary according to the profiles and specialties of the respective university. Stu-dents are advised to acquaint themselves thoroughly with the Medical Licensure Act and university study regulations. Both can be found on the webpage of the Faculty of Medicine student body of the University of Tbingen at http://fachschaftmedizin.de (in German only). Particular note should be taken of paragraphs 2 and 27, AppO. Recent policy changes to the AppO in the preclinical and the clinical semesters in particular have led to greater clinical orientation and stronger integration of preclinical and clinical training, increased emphasis on problem-based and evidence-based learning, the application of ethical com-petence, the teaching of communication skills, interdisciplinary lecture courses and elec-tive courses. AppO regulations have been implemented by the Tbingen Medical Faculty since the winter semester of 2003/2004. Two complete programs of study are offered at the Faculty of Medicine of the University of Tbingen. I. Medicine (Humanmedizin) This program terminates with the national licensing examination, which is a prerequisite for practicing as a physician. II. Dentistry (Zahnmedizin) This program is also completed with a national licensing examination conferring the li-cense to practice as a dentist. It is described in a separate brochure. 2. The Study of Medicine: application and admissions


The study program medical sciences belongs to the competitive numerus clausus subjects. Applications are processed by a national selection committee, the ZVS (Zentralstelle fr die Vergabe von Studienpltzen), Sonnenstr. 171, 44173 Dortmund. The primary selection criteria are high A-level marks (Abiturdurchschnittsnote, or equivalent), and/or position on the waiting list. More than half of all medical school slots are assigned by the selection committees of the universities themselves. However, the university selection process is accessible only via application to the ZVS, independently of the admissions criteria set by the individual universities. From among the applicants who are not admitted as students through the grade evaluation process, the waiting list or via status as a hardship case, the ZVS selects candidates in what is known as the university selection process (Auswahlverfahren der Hochschulen, AdH). University places left vacant are assigned by lot. The ZVS is responsible for German and EU applicants along with those from Iceland, Lichtenstein, Norway, and other foreigners and stateless applicants who have obtained German qualifying degrees (Hochschulzugangsberechtigung). The application situation differs for the individual universities. On the average, the University of Tbingen has 17 to 20 applications for each university place in medicine. The national average for all German medical is approximately 10 to 12 applications per slot. Thus, Tbingen ranges near the top of the national applications statistics, and takes first place in the state of Baden-Wrttemberg. Admission to medical school is possible twice a year in Tbingen, in the winter semester and in the summer semester. Currently, 318 students are admitted to their first semester every year, half in the winter, half in the summer semester. The entire medical student body in Tbingen is just over 2,500 persons. 

Study of Medicine

Every year the application deadlines of both ZVS and ABZ are January 15th for the following summer semester (beginning by mid April), and July 15th for the following winter semester (beginning by mid October). For candidates who received their qualifying degrees in year(s) prior to the year of application (socalled Alt-Abiturienten), the deadlines are May 31st for the winter semester and November 30th for the summer semester. 3. Orientation events for new students and students in their first clinical semester: You are not alone in the big university!
Beginning students who have been admitted to the University of Tbingen receive, together with their registration materials from the university, a detailed handbook (Guidelines for Students starting their Medical Studies) describing the first and second semesters. The guidelines include information about courses and registration for course placement. Additionally, during the week before commencement of classes, the Medical Faculty offers an introductory event with a tour of the city of Tbingen in cooperation with the Medical Students Association (Fachschaft). Participation in this event is strongly recommended. The exact times and places are announced in the student handbook. As part of our tutor program, professors and lecturers organize small medical student groups in order to advise them during the length of their studies. Another introductory program and handbook are offered to students to guide them through the clinical phase of the studies (Guidelines for the Clinical Phase of Medical Studies). 5. University courses
5.1. Tbingen study regulations and outlines of study
The study regulations and outlines of study are not identical at all German medical univer-sities. The current study plan in Tbingen is based on the National Medical Licensure Act (AppO), the valid university regulations and the study regulations in force since the winter semester 2003/2004. The Medical Facultys study regulations and outlines of study define the qualifications demanded of students and future graduates and doctors so as to edu-cate them to become scientifically trained and competent problem solvers skilled in diag-nosis, prognosis, therapy and ethics, as well as in practical communication. Knowledge and skills are not passively absorbed during standard lecture-style classes, but actively grasped and applied in small teaching groups. In addition, students are strongly encouraged to attend preparatory or complementary lectures which are integrated into the semester schedule. The schedules are posted in the Deans Office, and are also available in the Guidelines for Students Starting their Medical Studies as well as in the Guidelines for the Clinical Phase of Medical Studies. They can also be accessed on the internet under the electronic registration system SIMED (www.medizin.uni-tuebingen.de/en/Students/Study_of_Medicinep-1784/SIMED.html). The nationally binding AppO (current version dated 27th June 2002) stipulates the topics and courses (practical exercise labs, courses and seminars), which are to be successfully completed in the preclinical phase. In addition, students must complete an elective. For the clinical phase the AppO lists 21 subjects and 12 cross-sectional courses which are to be completed with a graded course credit (Leistungsnachweis), during the period between the First National Medical Licensing Examination and the beginning of the practical year (PJ). Additionally, a graded elective is required. For each of the individual subjects and cross-sectional courses the study regulations set forth curricular, compulsory-attendance practical labs, courses, plenum lectures and semi-nars. Overall subject grades may be made up of two or more single grades to be achieved in different semesters. Entrance into the practical year, which is a prerequisite for registration for the Second Na-tional Medical Licensing Examination, requires the successful and consistent attendance in all mandatory courses and the submission of the corresponding grades and credits and completion of the required minimum number of registered semesters.

5.4 Registration for courses and practical labs


Students in the preclinical and clinical semesters register for courses online through the student information and registration system SIMED. In order to register using SIMED stu-dents need a personal identification code which can be obtained in the Student Deans Office (Studiendekanat). The student must apply for the code in person and present his or her university transcript folder (Studienbuch). Deadlines for registration are published on the internet (http://www.medizin.unituebingen.de/en/Students/Study_of_Medicine.html) or posted on announcement boards in the Student Deans Office. Sign-up for preclinical electives, however, is done directly with the course instructor of the respective department. In the clinical semesters, students are assigned to groups in which they complete the required courses. Requests for assignment to specific groups are generally granted if space is available. The information needed for registration is sent to the student from the Deans Office via e-mail. For this reason, students are strongly advised to provide a valid email address.

6. Innovations in teaching at the University of Tbingen


The Medical Faculty of Tbingen constantly adapts to advances in teaching, actively implementing innovations and responding to student-faculty evaluations. Particular attention is paid to the close combination of preclinical and clinical teaching and interdisciplinary training in the preclinical and clinical years. Teaching quality 11

control is primarily carried out via train-the-trainer seminars offered to lecturers at the Competence Center for University Teaching in medicine (Kompetenzzentrum fr Hochschuldidaktik in der Medizin, http://www.mediadaktik.de) and via course evaluations (tuevalon, Tbingen Evaluation Online). Innovations in teaching Next to the regular courses and seminars one of the most crucial and successful highlights of medical studies in Tbingen is certainly the system of integrated seminars. These take place in the preclinical phase of study and include patient presentations. In this way, the traditional major subjects of anatomy, biochemistry, and physiology are coupled with early exposure to clinical practice. The new integrated lectures which take place during the sec-ond, third and fourth semesters have the same focus. After a six-week preparation period (specified preparation program with associated lec-tures, courses and practicals) in the first and second clinical semesters, medical courses are interdisciplinary (interdisziplinre Klinische Curricula, or i-KliC). i-KliCs are organized as week-long blocks in small groups and characterized by disease- and symptom-oriented topics. Their content and timing is aligned with mandatory courses and practicals. The goal is to develop diagnostic skills early on during medical studies. As a core element of the Tbingen study regulations, i-KliCs are progressively introduced in the 3rd to 6th clinical semesters. In the clinical phase medical students also take part in twelve mandatory crosssectional courses, each with an interdisciplinary focus. Building upon the basics of the i-KliC courses, students receive interdisciplinary training in the diagnosis and therapy of complex disease patterns relevant to comprehensive medical care. Further courses, the Tbinger Curricula Klinische Specials (TKliS) as well as the Tbinger Curricula Klinische Forschung (TKliF) are offered as electives, exposing the student to relevant patient-oriented aspects of the theory and practice of diagnostics, therapy and research. At the same time these courses serve to improve students communication skills and ethical competence, and allow students an upclose, personal look at real life as a physician. Clinical training is largely organized in units (Blockunterricht) including lectures, courses and practicals. Units span one, two, or three-week sessions. A two-week unit course in general medicine, which is attended before the beginning of the practical year, offers students the opportunity to become acquainted with the responsibilities and tasks of a general practitioner. The possibilities for e-learning are continuously integrated into the courses and practicals. Lecture and plenum contents can be viewed on the homepages of the individual departments.

7. Practical clinical training prior to and during medical studies


7.1. Patient care internship
Prior to the commencement of medical studies, or between course blocks in the preclinical phase, students are required to complete a patient care internship with a total duration of three calendar months, or at least 90 days, before registering for the First National Medical Licensing Examination. Students may divide this into a maximum of three parts, each part lasting at least 30 days. The goal of this internship is to introduce the prospective or current student to the operation and organization of hospital life and to expose him/ her to the routines of patient care. The internship must be performed in a governmentrecognized hospital. AppO paragraph 6 details the activities which qualify as patient care hospital work. 7.2. Clinical internship (Famulatur)


During the clinical phase of medical studies, students must complete several full-day internships during the out-of-session periods between course blocks. By taking part in inpatient and outpatient clinical routine, students are able to put to practice the knowledge acquired in their studies, most particularly the skills and proficiencies central to the medical profession. With this goal in mind, clinical internships (four months in total) complete the curricular clinical practicals. Two of these four months must be completed directly in the hospital and one month in the outpatient operations of a general practitioners office or in a public health facility. The final month can be done in either of these institutions. The clinical internship can be divided into no more than five sections, each section lasting at least 14 days. It can be completed in a foreign country, but the same rules apply as for domestic clinical internships. The forms for clinical internships are available in German, English, French and Spanish, and must be submitted when registering for the Second National Medical Licensing Examination.

Practical training

7.3. Practical training year (Praktisches Jahr)


The last clinical phase of medical studies, known as the Practical Training Year (Praktisches Jahr, or PJ) is a full-time clinical practical training covering a total of 48 weeks. It takes place during the last year of medical studies. It can be begun only after all of the pre-requisite coursework, practicals and internships have been completed and submitted. The PJ includes 16 weeks of practical experience in internal medicine, in surgery, and in general medicine or in an elective specialisation. It can be performed in a university hospital, in an approved and registered teaching hospital (Akademisches Lehrkrankenhaus, ALK) or in an approved and registered general practitioners office. The PJ at the Medical Faculty Tbingen is accompanied by a logbook which coordinates the subjects in each of the teaching hospitals. The topics entered in the logbook must be signed off by the respective instructor. The logbooks are to be submitted to the Student Deans Office (Studiendekanat) for inspection. The Faculty of Medicine assigns students to designated training posts. The PJ aims to incorporate the student into the routine of hospital operations. The goal is for students to apply and deepen their accumulated knowledge, skills and proficiencies during the PJ. In being exposed to the individual disease case, students gain insight into disease management combined with doctor-supervised diagnosis, therapy and patient care. Likewise, they learn to confer on pathological and pharmaceutical diagnosis and treatment. Parts of the PJ may be completed in a foreign country subject to approval by the Student Deans Office.

8. Examinations (National Licensing Examinations and university course credits)


8.1. First National Medical Licensing Examination (1. rztliche Prfung)
After the fourth semester at the earliest, a student can take the First National Medical Licensing Examination. This exam consists of a written part over a period of two days (four hours per day), and an oral/ practical part. The written part covers physics and physiology, chemistry and biochemistry/ molecular biology, biology and anatomy as well as the basics of medical psychology and sociology. The oral/ practical examination takes place in groups of up to four students before a twoperson commission and lasts between 45 minutes and one hour per examinee. Successful completion of the First National Medical Licensing Examination is the prerequisite for entering the clinical phase of medical studies. Clinical phase courses cannot be completed and recognized until this examination is successfully passed. 1

8.2. Second National Medical Licensing Examination (2. rztliche Prfung)


The student can sit for this examination a minimum of eight semesters after completing the First National Medical Licensing Examination (within the planned 12 semester medical studies track) and after completing the practical year and submitting the required university course credits. The examination comprises a written part lasting three days (five hours each day) and an oral/practical part. The oral/practical part takes place over two days. In groups of four examinees on each of the two days, the exam lasts between 45 minutes and 60 minutes per examinee. The practical examination includes a patient presentation. This examination covers internal medicine, surgery and a PJ elective. The Second National Medical Licensing Examination is the most comprehensive and most important examination in the course of medical studies. In order to calculate the overall grade, the grades from the written and oral practical parts are added together and divided by two.

8.3. Medical License Examination Diploma


The corresponding state agency (Landesprfungsamt) calculates the final grade for the National Medical Licensing Examination as follows: The numerical value from the First National Medical Licensing Examination and the double value from the Second National Medical Licensing Examination are added together and then divided by three. The final grade is computed to two decimal places (i.e. 1.35, 2.75, etc.). The university course credit grades are not counted towards this final grade, but are listed on the back side of the diploma.

8.4. General information regarding National Medical Licensing Examinations


The written multiple-choice examinations are held nation-wide in March and August each year for the first licensing examination, and in April and October for the second licensing examination. The examination topics to be covered are regulated in the German Medical Licensure Act (AppO). The oral/practical examination of the first licensing examination takes place in the out-of-session period between course blocks. In the second licensing examination, the oral/practical exams take place from April through June and from October through December. If a student does not pass an examination, he or she may retake it up to two more times.

Examinations

Registration for the examinations must be completed no later than the 10th of January or the 10th of June by submitting the required forms and university course credits (Leistungsnachweise) to the State Agency for Examinations in Medicine and Pharmacy (Landesprfungsamt fr Medizin und Pharmazie), as described in the AppO regulations. The examinees are invited by the agency to sit for the examinations and their results are processed by the agency and then sent to the students.

8.5. University examinations


During the preclinical phase, only the elective subject must be a graded credit. This preclinical elective may be selected from the entire range of university courses. The goal is to allow students investigate a subject of their choice beyond curricular medical training. The Medical Faculty of the University of Tbingen allows all preclinical courses that are not part of the required medical curriculum as possible electives. Upon approval from the respective instructors, lectures and seminars from any University of Tbingen department can be chosen. The student is responsible for independently selecting his or her elective and clarifying course details with the respective lecturer. He or she must also ensure that the elective covers the required minimum number of contact hours (at least 20), that the coursework is evaluated and a grade issued. Among the required university-graded course credits during the clinical phase, 21 subjects are listed in the AppO, of which at least nine are to be combined with two more subjects (interdisciplinary course credits, fcherbergreifende Leistungsnachweise). Further requirements are course credits for one elective, 12 cross-sectional subjects, and five compact practicals (Blockpraktika) in internal medicine, surgery, pediatrics, gynecology/obstetrics, and general medicine. During the clinical semesters, final examinations are held at the end of each semester (Zentrale Prfungen). Theoretical topics are tested in a written examination. Oral/practical tests are conducted periodically in order to examine the students performance in the practical labs,. Overall university subject grades may be made up of two or more single grades. The AppO specifies a comprehensive list of possible clinical electives (minimum elective length is 40 contact hours, to be completed during the clinical phase of the studies). The range of electives may vary from university to university. All university grades (12 subject grades, 3 interdisciplinary subject grades, 12 cross-sectional course grades, 5 compact practical grades) are recorded separately. They do not, however, count towards the final national license grade.

9. The profession of medical doctor and continuing education


Full licensure as a physician entitles one to independently practice the profession either as an employee or as a solo practitioner. Application for licensure is processed by the responsible state agency (of the Federal State in which the applicant has passed the Second National Medical Licensing Examination). At the present time, solo practitioner licensure is limited. Admittance as a doctor within the public health system (Kassenarzt) requires a period of continuing education complementing the national license exam. After this period of continuing education, the doctor is entitled to be orally examined by the Medical Association (rztekammer) to be certified in a medical specialty such as general medicine, surgery, pediatrics, etc. The continuing education period for such specialization can take up to six years or more. In their corresponding continuing education regulations, the Medical Association has stipulated which medical training is necessary in order to qualify for each specialty. The state and county Medical Associations are responsible for all questions pertaining to continuing education.

If a student wants to continue his/her medical studies at another German university, he/ she must apply to and be accepted by the other universitys medical faculty. (16) Trial study period, guest student status


The University of Tbingen offers a wide range of courses for those who do not wish to register as full-time students at the university. As a student, you have access to the university`sBelwue wifi network, with hotspots on the university campus as well as in the Students Dean`s office (Geissweg 5/1), the Lehr- und Lerngebude (Elfriede-Aulhornstr.), the Old and New Anatomy buildings, in and around the auditorium of the Crona Hospital, in the Dept. of Internal Medicine, as well as in the Institute for Occupational and Social Medicine (Wilhelmstr. 21). The necessary VPN client is available after registration with the university computer center ZDV.
