Student Matinee Order Form and Subsidy Applications

GUIDELINES Please do not request multiple titles on one form. One order form and one ticket and/or transportation subsidy application is required for each show title requested. Reservations made that are not approved for either subsidy can be declined. The Ticket Subsidy cannot be applied retroactively to closed or paid orders. A Transportation Subsidy Application must be submitted prior to the performance date. TICKET SUBSIDY ELIGIBILITY Eligibility will be determined using classroom percentages of students enrolled in free and reduced lunch programs. % of F/RL Student Population 30-55% 56-79% 80-100% Discounted Amount* $2.00 off per ticket $3.00 off per ticket $4.00 off per ticket

TRANSPORTATION SUBSIDY ELIGIBILITY Schools must fall into one of the two following categories to apply for funding: 1. Schools whose student population consists of 50% or more of the student population being eligible for free or reduced price school meals (F/RL) (Up to $200/trip). OR 2. Schools located 75 miles or more from the theatre (Up to $300/trip). TRANSPORTATION COST REIMBURSEMENT After their performance Educators who have received a 2012-2013 Transportation Subsidy must submit a Reimbursement Request form and proof of payment to School Group Sales to receive reimbursement. Reimbursement request forms can be found online at www.childrenstheatre.org. All reimbursement requests must be submitted to School Group Sales by June 21, 2013. Reimbursement will be made within 30 days of receiving all required materials. TICKET AND TRANSPORTATION SUBSIDY APPLICATIONS An application must be submitted to be eligible for Ticket Subsidy and/or Transportation Subsidy funding. Grants will be given on a first-application-received basis until funds are gone CONFIRMATION/DENIAL OF APPLICATION Educators will be notified of status within two weeks of submission. A reservation contract will be sent for all ticket reservations. Approved Ticket Subsidy applications will receive a ticket subsidy reservation contract and invoice. Approved Transportation Subsidy applications will receive a confirmation e-mail that includes the reimbursement procedure. Denied subsidy applications will be placed on a waiting list and will be contacted in the order that they were received if additional funds become available.
