This manual establishes the guidelines with respect to production and reproduction of documents and the acquisition of print related services. It outlines the roles, responsibilities and accountabilities for all printing services. The manual applies to all document printing, design and other print-related services including photocopying, laser printing (color and black and white) design or any other related services.

IMPORTANT: This manual is an internal document of MQCPI and as such deemed strictly confidential and private. Unauthorized replication or distribution of this manual regardless of purpose is prohibited and non-compliance shall be dealt with due process.

B. Printing Office Assistant Job Description 1. Job Summary: 1.1 Primary duties include preparation, operation, and maintenance of printing services. 2. Duties and Responsibilities: 2.1 Formulates and implements printing policies, guidelines and procedures. 2.2 Schedules and prints test manuscripts. 2.3 Orders and purchases of printing supplies. 2.4 Prepares reports on inventory and issues necessary memorandum for preventive and corrective measures. 2.5 Performs regular maintenance every month and minor troubleshooting of printing equipment. 2.6 Manages, operates, maintains, supervises and secures the process and activities within the office. 2.7 Ensures security and safety of equipment and documents. 2.8 Processes request for printing from non-teaching personnel, faculty, heads and executives of the college.

C. General Policy 1. The Printing Office Assistant (POA) is responsible for the management, operation, maintenance, supervision and security in the process and activities within the office. 2. Only the printing office assistant is allowed inside the office. Students and teachers who have no permission from the POA/Administrative Group Head or without official business are prohibited from entering and staying in the office. Violations are subject to appropriate disciplinary sanctions. 3. Office equipment/machines such as copiers, printer, computer, etc. are for official use only, and only the printing office assistant or duly authorized personnel is allowed to operate and use the equipment. 4. Maintenance of printing office equipment should be regularly observed. 5. Security and safety of equipment and documents should be strictly observed. 6. All personnel should accomplish the necessary Printing Request form for any printing request. Refer to Annexes. D. Services 1. General Printing of Documents 1.1 Guidelines for general printing requests 1.1.1 The printing office offers printing services to all personnel of the college. However, only official and approved documents are allowed to be printed. 1.1.2 All requesting personnel are required to accomplish the General Printing Request Form. Refer to Annex A General Printing Request Form. 1.1.3 Documents produced in large numbers (more than 20 pages) are subject for approval from their respective department/group head. Documents with 19 pages or less are to be logged in a record book. 1.1.4 Documents must be in their final form. Editing of documents inside the office is strictly prohibited. 1.1.5 Requesting personnel must submit the document via electronic mail, flash drive or any other external memory devices. 1.1.6 Only the requesting personnel is allowed to claim the requested documents. In case of absence, the representative must secure an authorization letter from the requesting personnel. 1.1.7 In case of reprinting due to erratum or correction of previously printed documents, the reprinted documents will still be charged to the requesting department. 1.1.8 Unclaimed printed documents will still be charged to the requesting department.

Printing of colored documents such as images, clip art, etc. is subject for approval from the department head.

2. Major Examination Printing 2.1 Guidelines for major examination printing requests 2.1.1 To minimize the possibility of leakage, the printing office is the only authorized office in printing copies and reprinting of examination materials. 2.1.2 The printing assistant will route a Memorandum (Refer to Annex E), Sample Test Paper (with approved format) (Annex F) and distribute the Examination Printing Form (Annex B) to instructors, department heads and coordinators in preparation for the major examinations. Preparation includes requesting of printing materials, checking of printing machines and drafting of test questions for the instructors. 2.1.3 Instructors must submit the draft of the examination manuscript (hard copy with the correct format) and fully accomplished Examination Printing Form (Annex B) to their department heads/coordinators 15 working days before the first day of the scheduled examination period. In case of non-submission of examination manuscript, the instructor must fill-out a Non-submission of manuscript form (Refer Annex C) that can be obtained from the Vice-President for Academic Affairs (VPAA). Submission of Non-submission of manuscript form is 5 working days before the examination date. 2.1.4 Department heads/Coordinators must check the content and format of the examination manuscript. Erratum must be corrected by the instructor and rechecked by the department heads/coordinators. 2.1.5 Collection of examination manuscripts from the department heads/coordinators is 5 working days after the scheduled submission of examination manuscripts. 2.1.6 In case of Re-scheduling of Examination, the instructor must fill-out the Reschedule of Examination Form (Refer to Annex C). 2.1.6.1 If the new schedule is earlier than the original schedule, the instructor must submit his manuscript 15 working days before the new schedule to the department head/coordinator. Afterwards, the department head/coordinator must submit the approved examination manuscript with the Reschedule of Examination Form to the Printing Office five (5) days before the scheduled examinations. 2.1.6.2 If the new schedule is later than the original schedule of examinations, the instructor should still submit the manuscript with the Reschedule of

Examination Form to their department heads/coordinators, fifteen (15) working days before the first day of the scheduled examination period. 2.1.6.3 No rescheduling of examinations after the scheduled submission of manuscripts to the department heads/coordinators will be allowed unless approved by the Vice President for Academic Affairs. However, printing of said examination manuscripts will be least prioritized. 2.1.7 Failure to submit on the scheduled deadline will be subject to appropriate sanctions. Late submitted manuscripts will be printed unless authorize by the Vice President for Academic Affairs. However, printing of the said examination manuscripts will be least prioritized. In case of the absence of the Vice president for Academic Affairs, the department heads/coordinators will take charge. 2.1.8 Involvement of students, faculty members, outsiders in the preparation of examination manuscripts is strictly PROHIBITED, only authorized personnel is allowed inside the printing office. 2.1.9 Instructors, institute heads/coordinators and the printing office assistant are accountable for the confidentiality and security of the examination manuscripts. 2.1.10 During the preparation of examination manuscript, the instructors will be the only authorized person to define the scope of examination, formulate the questions, type or re-type the manuscript, edit the manuscript. In case of departmentalized examination, only the instructors involved in the subject area are allowed to access any documents or manuscripts concerning major examinations. 2.1.11 Only the respective instructor and/or their institute heads/coordinators and the printing office assistant will be allowed to have access to the examination manuscript. Unauthorized personnel and students are not allowed to access any documents or manuscripts concerning major examinations. 2.1.12 Institute heads/coordinators must monitor and check the preparation of examination manuscript. The institute head/coordinator is the only authorized person to secure the examination manuscript envelope. 2.1.13 The printing office assistant is the only authorized person to gather or receive the examination manuscript envelopes from the institute heads/coordinators which will be collected or submitted five (5) days after the scheduled submission of examination manuscripts. 2.1.14 During the printing of the examination manuscript, only the printing office assistant is allowed to enter the printing office. Only authorized personnel are allowed inside the printing office. Instructors and Institute heads/coordinators are allowed to enter the office only during submission and claiming of printed

examination manuscripts or for other concerns regarding the examination manuscripts. 2.1.15 When the examination manuscripts are already submitted, the printing office assistant will stamp the examination manuscripts as received. Printed examination manuscripts with no stamp from the PO means the examination manuscripts was not printed inside the PO. 2.2 Leakages 2.2.1 In case of leakage, the Printing Office in coordination with the Office of Student Affairs (OSA) will conduct an investigation. 2.2.2 Once the leakage is confirmed, the following sanction will be taken into consideration: a. Student(s) involved in the leakage will be required to report to the Office of Student Affairs. b. Teacher(s) involved in the incident will be required to report to the Vice President for Academic Affairs. c. Non-teaching employee(s) involved in the incident will be required to report to their respective group heads. 2.3 Sanctions 2.3.1 Instructors violating the guidelines set forth in this manual will be subjected to disciplinary actions. a. Sanctions for teachers who failed to submit examination manuscripts on time: 1st Warning/Memorandum 2nd The teacher will be asked to shoulder the expenses for the printing of the documents. 3rd - The teacher will be asked to shoulder the expenses for the printing of the document and will be asked to report to the Vice President for Academic Affairs.
