Contract of Agreement

Any changes deemed inappropriate will automatically terminate this agreement. At which time the client will forfeit any outstanding monetary percentage profits in his/her account.

Rules & Regulations: All rules follow the guidelines laid out

and agreed upon by union standards of SAG, MMNY & the US child Labor law guidelines & are in accordance with the contract and bilaws set forth for the operating year 20___. Any disputes must be sent in writing to the agency within 30 days of said contract disagreement. a. Clients reserve the right to refuse any assignment with a feasible argument. Notice must be in writing no later then 10 days prior to the assignment. Clients will be responsible for refunding any pre paid fees and or wages at this time. b. Clients may not partake of any food, beverage or tobacco product in the presence of any account holder. c. Clients must not use any illegal substance. Doing so will automatically terminate this agreement. Clients may be asked to submit to drug testing at his/her own expense if they are believed to

have ingested in any form or found to be in possession of said substances at any time. Failure to comply with the written request within 24 hours will terminate this agreement and at which time the client will forfeit any outstanding monetary percentage profits in his/her account. d. Clients must contact the agency, Lady Cassadys LLC for confirmation the day prior to an assignment, upon completion of an assignment & once every thirty (30) days to uphold and validate this agreement.

Photography & Portfolios: photography rules include any

and all existing photographic material regardless of photographer and date of exposure. Portfolios include all photographic, video, & digital imaging in existence that in any way portrays & or displays the image or associated image of the client. a. Every client over sixteen (16) years of age must have a current portfolio that meets the standards set forth by union standards of SAG, MMNY & the US child Labor law guidelines & are in accordance with the contract and bi-laws set forth for the operating year 20___. The portfolio must include the highest quality photogenic materials shot by an experienced & professional photographer. b. Clients under the age of seventeen (17) years of age must update the portfolio every thirty (30) days while under this agreement with the highest quality photogenic materials shot by an experienced & professional photographer. c. Photogenic materials, video and digital portfolios must be made available within 48 hours for clients when requested. d. A list of approved photographers will be made available to the client if pictures are deemed necessary. It is in your best interest to use these photographers whenever possible to avoid inappropriate material, over charging, unsuitable or poor quality photographs.

Digital portfolios are not required but it is the understanding of the client that those clients that obtain and maintain a digital portfolio will be more accessible to a wider range of clientele/account holders. f. All clients under the age of eighteen (18) are required to have in there possession an up to date color head shot. Copies must be made available within 48 hours upon written request of the agency. Photos must be exact representation of client on the day of audition. Pageant composites are not acceptable.

I understand although I am free to free lance under the terms of this agreement it is considered a professional courtesy to inform the agency of any free lance work, date Time and wages included. Clients may not free lance for any open account holder of Lady Cassadys Finishing School LLC within 2 years of receiving monetary compensation for services through
