SUMMARY: Patch Adams is a true story of Hunter "Patch" Adams, the founder of the Gesundheit Clinic, a clinic which deals with their patients with humor and pathos. The film starts with Hunter Adams (Robin Williams) a troubled man, who voluntarily commits himself as a patient in a mental ward at Fairfax Hospital. While there, he finds that he enjoyed helping the other patients and earned the nickname "Patch". He also found out that the staff is cold and separative from the patients. Vowing to change things, he releases himself from the hospital and headed to the Virginia Medical University, being the oldest first year student. He questions the school's approach to medical care, which has a sickeningly callous philosophy that advocates an arms-length attitude to the patients that does not address their emotional needs or the quality of their lives. Thus, Patch Adams is determined to find a better way to help patients. This causes him to always clash with the school's Dean Walcott (Bob Gunton), who believes that doctors must treat patients and not bond with them as people. Because of his contradiction to the Dean and certain incidents like a prank during a medical conference where he was assigned to decorate the place and he put giant legs with a caption At your Cervix, he is expelled from the school, although he is later reinstated due to his methods actually improving the health of patients. Patch encourages medical students to establish a good working relationship with nurses and treat them as equal level to them and learn their interviewing skills early for them to give holistic care to the patients. He also argues that death should be treated with dignity and even humor. Adams begins a friendship with fellow student Carin Fisher (Monica Potter), and develops his idea for a medical clinic built around his philosophy of treating patients through humor and compassion. With the help of Arthur Mendelson (Harold Gould), a wealthy man who was a patient whom Patch met while in the mental hospital, he purchases 105 acres (425,000 m) in West Virginia to construct the future Gesundheit! Institute. Together with Carin, medical student Truman Schiff (Daniel London), and some old friends, he renovates an old cottage. When they get the clinic running, they treat walk-in patients without medical insurance, and perform comedy sketches for them. Carin and Patch's friendship soon turns into romance. She reveals to him that she was "attracted by men" her whole life, distancing herself from men and dreaming of becoming a butterfly to escape her torment, but Patch reassures her that she can overcome her pain to help others. Encouraged, Carin wants to help a disturbed patient, Lawrence "Larry" Silver (Douglas Roberts), by visiting him at home. Larry turns out to be far more disturbed than anybody expected, and murders Carin, turning the shotgun on himself soon after. Patch is devastated and guilt-ridden by Carin's death. He reconsiders his outlook, and questions the goodness of humanity. He contemplates suicide and questions God for what happened. He then turns to see a butterfly which reminds him of Carin and hope, and so decides to continue his work in her honor. However he is dismissed from the school for a second time because he ran a clinic and practiced medicine without a license. He files a grievance with the state medical board at the advice of his former Med school roommate, Mitch Roman (Philip Seymour Hoffman), and must attend a hearing where it is decided whether he can graduate. Patch is able to convince the board in a final speech that he did his best to help the people that came to him, and as a doctor it is his responsibility to treat the disease of the body as well as a person's spirit regardless of outcome. The jury accepts Patch's medical methods and decides to allow him to graduate. He receives a standing ovation from the packed hearing room. Even at graduation, Patch cannot help but be a non-conformist. After handing Adams a diploma, Walcott approves of Patch's conformity, to which Patch bows to the professors and the audience, revealing his nude bottom under his gown.

PSYCHOTHERAPEUTIC INTERVENTIONS Psychoanalytic Therapy SPECIFIC SCENARIO When Patch Adams was talking to the Psychiatrist in the Fairfax Hospital. ANALYSIS (DOCUMENT) Adams stating his experiences when he was still nine years old. His father who was an army isnt home that much. His father said that during the Korean War, he felt like he lost his soul. And now, Hunter Adams feels like he lost his soul too. During the previous year, he has transferred from one place to another and had several jobs. Despite these, he doesnt feel the devotion or commitment to what he is doing. There were five patients in this session. Patch, Rudy (his roommate), Bile (catatonic) and the other two patients. This session turned to be hilarious because Adams told the psychiatrist that Bile maybe wants to participate in their discussion. After that, his fellow inmates laugh out loud. The doctor got frustrated and ended up the session. Patch used things like the suctioning bulbs and put it in his nose so he would look like a clown. He did funny things that caused the children to laugh hard. Although Patch is not yet allowed to interact with the patients because he is only on the 2nd year and they have a rule that medical students are not allowed until the 3rd year. So, when the Dean found out about this, Patch was called and warned about what he did. Although warned, Patch continued to use his way of helping patients by his excessive happiness and compassion to his patients. The nurses came in when Patch called for help. They injected a medication on Rudy to make him calm and sleep.

After his interaction with the psychiatrist, the patients in the mental ward were gathered and they had a group talk.

When Patch visited the cancer patients in the childrens ward.

Patchs roommate Rudy who experienced delusions about the squirrels who want to kill him.

CHARACTER ANALYSIS: CHARACTER Hunter Patch Adams SIGNIFICANT BEHAVIOR Repeatedly and impulsively suicidal, hes excessively happy and quick minded or very sad and down, when hes in love he almost harrasses the girl (Carin Fisher) like a stalker. He seems to want to escape his fears and insecurity by manically positive thinking. DISORDER ANALYSIS (DOCUMENT) Pre-suicidal syndrome The Presuicidal Syndrome, first described in 1958, relates to that specific psychic state of mind that leads to suicidal acts. It is an alarm signal indicating a tendency to suicide. The Presuicidal Syndrome has three principal components: constriction; inhibited aggression turned toward the self; and suicidal fantasies. In the story, Patch attempted to commit suicide by jumping on a cliff because of guilt and devastation of Carins death. He questioned God for what happened. But luckily, he was enlightened again once he saw a butterfly that remind him of Carin and this inspired him to continue his devotion of helping people. Post Traumatic Stress Carin was molested as a Disorder (PTSD) child that is why she distanced herself from men. She told Patch that she dreamed of becoming a butterfly. Patch helped him to trust and forget her fear by helping others. Catatonic Patient Bile exhibited the stereotyped position for catatonic and extremely mute.

Carin

Critical and easily irritated, depressed and task focused, unstable interpersonal relationships, selfimage, identity, and behavior.

Paranoid schizophrenia

Patient Larry killed Carin when she went to his house with the purpose of helping him. After killing Carin, he then shoots himself.
He taught Patch to look at things in a way other people dont; more deeply, and to not focus on the problem. Rather, focus on the solution.
