Ceremonies by JodieLyn & Associates greatly appreciates your business. Please fill out this questionnaire,
as it helps me to determine a fee, and how I can personalize your ceremony. Please get it back to me as
soon as possible so we can add you to our calendar. Please fill it out on your computer, save it, and then
reattach the document to an email. You can also print, fill it out and mail it to me, or print the document
and scan back in to email the forms back. If you fill it out on your computer, please fill out using RED &
highlight any part that you would normally circle.
This questionnaire will become a part of the contract/agreement once it is signed by both the client and
Jodie L Henslee- Soto, (dba Ceremonies by JodieLyn& Associates) until then it is a preliminary prewedding questionnaire for information purposes only. We will discuss more in depth questions that
you may have when a consult is arranged. Consults can be done in person, by phone, webcam or via
email, etc. Once hired, I am always available to answer questions; the best way to contact me is text or

SECTION H:
Upon the Bride and Groom deciding to hire Ceremonies by JodieLyn & Associates the
signing of this questionnaire then becomes an agreement and constitutes the hiring of a
Minister ordained by the ULC Ministries & a Minister with Ceremonies by JodieLyn &
Associates. Ceremonies by JodieLyn & Associates is a separate entity of ULC and AMA
but has been legally ordained by ULC and/or AMA and as such the officiant that performs
your ceremony is an ordained Minister/Officiant.
Ceremonies by JodieLyn & Associates has more than one minister working within the
company and will fit a minister to your preference and wedding style, so as one is
available. JodieLyn does all the ceremony writing personally, so that the ceremony is
perfect for you. Ceremonies by JodieLyn & Associates bear no legal or financial liability
to any individual attending this event that should be harmed or cause harm to another
person or property for whatever reason or cause. The client accepts all responsibility for
all guests invited and uninvited for injuries and/or property damage.
Ceremonies by JodieLyn allows a leeway of thirty minutes (30min) for the ceremony to
begin. Any time over that shall be charged at a rate of $15.00 per fifteen minutes that
we would have to wait to begin the ceremony. We have weddings and meetings that we
need to tend to and we try to schedule accordingly.
The Marriage License has specific instructions. If it is not done according to State
Standards, and is sent back for resigning by the officiant, and if it is not the fault of
Ceremonies by JodieLyn & Associates, (a mistake made by the bride, groom, or one of
the signing witnesses) then it will be up to the Bride and the Groom to pay the fee as set
forth by the county to which the license was returned by. Depending on the county will
depend on the fee charged. If a mistake was made by Ceremonies by JodieLyn &
Associates then this company will be responsible for paying the resigning fee.
This contract/agreement contains all terms, conditions and services agreed upon
by the client and Ceremonies by JodieLyn & Associates, once it is signed by both parties
and all terms are agreed upon. No other agreements, oral or otherwise regarding the
matter of this contract/agreement for the stated event shall be deemed to exist.
