List of Florida Board of Nursing Approved Remedial Courses (February 2010) These courses may be taken by applicants who

have failed the NCLEX three times or who have not had an active nursing license for the past five years and are applying for endorsement based on having passed the NCLEX. As of October 1, 2009, the Board office will only accept an original letter on official letterhead from the program director with the directors original signature as proof of program completion. This letter must be sent directly to the Board office by the program. Applicants should call the Board office at the following numbers if they require letters of authorization; Applicants who have failed the NCLEX three times will require a letter to begin the remedial course. To obtain a letter, please call 850-245-4125 or email MQA_Nursing@doh.state.fl.us. Applicants who do not have an active license, and are applying for licensure by endorsement based on having previously passed the NCLEX, will require a letter of permission to complete the clinical component of the remedial course. To obtain a letter of permission to begin your clinicals, call 850-2454125 ext. 3612 or email MQA_Nursing@doh.state.fl.us. These applicants will not need an initial letter of permission to begin the remedial course. o Please provide the name and location of your clinical site, the clinical rotation completion date and the name of your remedial program when requesting this letter.
