AchieveCard Platinum Plan Terms and Conditions

These AchieveCard Platinum Plan Terms and Conditions are effective as of March 9th, 2013 and are in addition to the terms and conditions contained in the Cardholder Agreement for the AchieveCard Prepaid MasterCard card. These AchieveCard Platinum Plan Terms and Conditions (these "Platinum Terms and Conditions") outline the terms and conditions of your AchieveCard Platinum Plan. By enrolling in the Platinum Plan, you represent and warrant to us that you received a copy of these Platinum Terms and Conditions and you agree to be bound by and comply with the terms hereof.

These Platinum Terms and Conditions are in addition to the terms and conditions contained in the Cardholder Agreement for the AchieveCard Prepaid MasterCard card (the "Cardholder Agreement") and are incorporated into and made a part of the Cardholder Agreement for Platinum Plan enrollees. The terms "we," "us," "our" and "AchieveCard" as used herein refer to Achieve Financial Services, LLC, a Delaware limited liability company. The term "Card" as used herein refers to the AchieveCard Prepaid MasterCard card; the terms "you" and "Cardholder" as used herein refer to a holder of the Card; and the term "Card Account" as used herein refer to a Cardholder's account for an AchieveCard prepaid card. Please read these Platinum Terms and Conditions carefully. Some of the benefits and services of the Platinum Plan are subject to important limitations.

Enrollment and Eligibility

Cardholders may enroll in the Platinum Plan at any time by calling 1-800-486-0273 or online at www.achievecard.com. Your enrollment will become effective on the first day of your next billing cycle. Your billing cycle is defined as every 30 days from date on which you first loaded funds to your Card. Therefore, you may have to wait up to 30 days for the Platinum Plan to take effect. Once your enrollment is effective, the features of the Platinum Plan, as described below under "Platinum Plan Features," may not be available to you for up to 72 hours for administrative reasons. You will receive a confirmation email as soon as the features are available, along with instructions on how to login to your Platinum Plan account online. Further, in order to take advantage of the Achieve ID Theft Protection Program services, you will need to login to your Platinum Plan account online and provide certain additional information to the third party provider of the Achieve ID Theft Protection Program, as described below under the description of this program.

Fees
The monthly maintenance fee for the Platinum Plan is $14.95 per month. All other fees applicable to the Platinum Plan, including, for example, for pin purchase transactions, ATM withdrawals and ATM balance inquiries, are identical to the fees assessed under our standard Gold Plan. All fees assessed by us in connection with the Platinum Plan are deducted from your Card Account available balance. The monthly maintenance fee will be deducted on the first day of each billing cycle. For other important information about the fees charged to your Card Account, please refer to the section in the Cardholder Agreement entitled "Important Information about Fees Charged to Your Card Account."

Cancelling or Switching Fee Plans

You may cancel the Platinum Plan or switch fee plans at any time by calling us at 1800-486-0273. If you choose to cancel, the cancellation will be effective on the first day of your next billing cycle. If you choose to switch fee plans, your new fee plan will become effective on the first day of your next billing cycle. We may cancel your Platinum Plan or switch your fee plan for nonpayment of fees. If you enroll in the Platinum Plan but we are unable to collect your full monthly maintenance fee on your latest billing cycle because you do not have a sufficient available balance, your Card Account will be drawn negative and your fee plan will be switched back to our standard Gold Plan if you do not reload your Card within the next 30 days with a sufficient available balance that allows us to collect any past due monthly maintenance fee. After this 30-day period, you will no longer receive the Platinum Plan benefits of the Achieve Discount Health Plan, Achieve ID Theft Protection and Achieve Roadside Assistance. We will also close your Card Account 90 days after the date of the first negative balance drawn, if your Card Account continuously maintains a negative balance from that date and/or we remain unable to collect past due monthly maintenance fees. For information about the potential cancellation of your Card for nonpayment of fees, please refer to the section in the Cardholder Agreement entitled "Important Information about Fees Charged to Your Card Account".
