DEFINITIONS

page defines the terms associated with aircraft accidents and incidents.

Accident - An occurrence associated with the operation of an aircraft which takes place between the time any
person boards the aircraft with the intention of flight and all such persons have disembarked, and in which any
person suffers death or serious injury, or in which the aircraft receives substantial damage.

Serious Injury - Any injury which results in a fracture of any bone (except simple fractures of fingers, toes, or
nose), or causes severe hemorrhages, or nerve, muscle, or tendon damage, or involves any internal organ, or
involves second or third degree burns, or any burns affecting more than 5 percent of the body surface.

Substantial Damage - Damage or failure which adversely affects the structural strength, performance, or flight
characteristics of the aircraft and which would normally require major repair or replacement of the affected
component. Engine failure or damage limited to an engine if only one engine fails or is damaged, bent fairings
or cowling, dented skin, small punctured holes in the skin or fabric, ground damage to rotor or propeller blades,
and damage to landing gear, wheels, tires, flaps, engine accessories, brakes, or wing tips are not considered
substantial damage for the purpose of this matrix.

Incident - An occurrence other than an accident, associated with the operation of an aircraft, which affects or
could affect the safety of the operation.

ULTRALITE ACCIDENT
5FINITIONS:

Ultralight Vehicle - An aeronautical vehicle operated for recreation or sport purposes only, which does not
require an ah" worthiness certificate, FAA registration, nor pilot certification. They are primarily single
occupant vehicles, although some two-place vehicles are authorized for training purposes. (Note: For the
purposes of this SOG, ultralight vehicle accidents will be treated as aircraft accidents)

AIRCRAFT INCIDENT
DEFINITIONS:

luvident - An occurrence other than an accident, associated with the operation of an aircraft, which affects or
could affect the safety of the operation. (Note: For the purposes of this SOG, ultralight vehicle incidents will be
treated as aircraft incidents).

Spill Out - An excursion of a military aircraft from the exterior boundary of Special Use Airspace allocated to
military using agencies into other controlled airspace without coordination or approval. For the purposes of this
SOG, Spill Out will be treated as a military incident. When making the Daily Operations Report (OR) entry,
annotate the words "Spill Out" after the date.

Response Plan, 4040.25, in the AEA-200 section of the orders book.
7. All incidents with the exception of an aircraft that lands and taxis to the ramp without incident or aborts
takeoff and returns to the ramp or subsequently departs without incident.

OVERDUE AIRCRAFT

EFINITIONS:

When a facility reports an aircraft is overdue, missing, or unreported, the ARTCC in whose area the
aircraft is first unreported, will determine the course of action required. The ROC will comply with
procedures contained herein when notified by an ATC facility that an aircraft is overdue, missing, or
unreported.

OPERATIONAL ERROR/DEVIATION

DEFINITIONS:

OPERATIONAL ERROR - An occurrence attributable to an element of the air traffic system which results
in less than the applicable separation minima between 2 or more aircraft, or between an aircraft and terrain
or obstacles, as required by Order 7110.65 and supplemental instructions. Obstacles include
vehicles/equipment/personnel on the runways.

OPERATIONAL DEVIATION - A controlled occurrence where applicable separation minima as described


above was maintained, but:
1. Less than the applicable separation minima existed between an aircraft and protected airspace
without prior approval.
2. An aircraft penetrated airspace that was delegated to another position of operation or another
facility without prior coordination and approval.
3. An aircraft penetrated airspace that was delegated to another position of operation or another
facility at an altitude or route contrary to the altitude or route requested and approved in direct coordination
or as specified in a Letter-of-Agreement, pre-coordination or internal procedure.
4. An aircraft, vehicle, equipment, or personnel encroached upon a landing area that was delegated
to another position of operation without prior coordination and approval.

REMARKS:
1.- When a facility notifies the ROC of NMAC Report, the Operations Officer should ascertain if a report has

NMAC report does not constitute such a report.
2.- If a pilot or flight crew member calls the ROC with the intent to file a NMAC report, have the person contact
the nearest Air Traffic facility at or nearest the flight's destination. The normal order of facilities to be contacted
is ATCT, FSS, ARTCC, and FSDO.

FAR ARREST

DEFINITIONS:

FAR ARREST - A report which usually involves a weapon discovered at the screening point, unlicensed
weapon in checked baggage at the ticket counter, unauthorized access to a restricted area such as the Air
Operations Area, an aircraft, a sterile concourse or a critical airport facility, making a false threat, etc. In
such situations, the airport police are called and a suspect is taken into custody, or a court appearance
summons is issued in lieu of making a summary arrest, for a violation of a law or regulation.

BOMB THREAT

EFINITIONS:

BOMB THREAT - Information received from any source that a bomb has been placed or suspected being
placed on, in, or near an aircraft, airport facility, FAA facility, navigational facility, or any other element of
the air traffic system critical to the safety of air traffic.

SPECIFIC THREAT - A threat against an aircraft identified in the threat by flight number, departure tune,
or actual location of the airplane at the time of the threat, when the text of the threat also includes "positive
target identification" (PTI).

POSITIVE TARGET IDENTIFICATION - Information regarding the target such as an "N" registration
number of an aircraft, a crew member's full name, a valid baggage tag number, industry-type nomenclature,
or any other normally nonpublic information included in a bomb threat indicating firsthand or unique
knowledge of targets.

RUNWAY INCURSION

takeoff, landing, or intending to land. Runway incursions result from one of the following four types of
airport surface occurrences: Pilot deviations, operational errors, vehicle operator/pedestrian deviations, and
pilot/vehicle operator/pedestrian judgmental errors.

causes death, injuries requiring hospitalization, estimated damage exceeding $50,000, radioactive or
etiologic contamination, evacuation of the general public occurs lasting one or more hours, the operational
flight pattern of an aircraft is altered, or an incident in which a discovery is made of an undeclared shipment
that actually contains a hazardous material/dangerous good, or a situation exist of such nature that, in the
judgment of the air carrier, should be reported to the DOT.

In an effort to assist the RETCO/RETREP in the accomplishment of their duties, the Federal Aviation
