Before me, the undersigned Assistant District Attorney of Harris County, Texas, this day appearedthe undersigned affiant, who under oathsays that he ha.s good reason to believe and docs believe that in Harris County. Texas. ANA M:AR.l'A GONZALEZ.ANGULO here after styled the Defendant, heretofore on or about JANUARY 27, 2013, did then and there unlawfully. inte:ntionally and knowingly cause serious bodily injury to GEORGE BLUMENSCHEIN, a person with whom the Defendant had a dating relationship hereafter sryled the Complainant, by POISONING TH.E COMPLAINANT with a deadly weapon, namely, ETHYLENE GLYCOL. PROBABLE CAUSE Macario Sosa, Jr.a peace officer employed by lhe Univcrsity of Texas at Houston Police Department, do solemnly swear that I have reason to believe and do believe that on or about January 27. 2013 at 3860 Gramercy St., Houston, Harris County, Texas, Ana Maria Gonzalez Angulo, a White Female. with a date of birth of February 9, 197 L , and whose last known address is 3860 Gramercy St, Houston, Harris County, Texas, committed the felony offense of Aggravated Assault against a victim with whom the actor has a dating relationship causing serious bodily injury, in violation, of the Texas Penal Code.

MY BELIEF IS BASED UPON THE FOLLOWING FACTS: .

Your Affiant is a peace officer with che University of Texas at Houston Police Department and Your Affiant was assigned to follow up on the investigation of a poisoning case that took place on January 27, 2013,at 3860 Gramercy St.,Houston, HarrisCounty, Texas. Your Affiant spoke with George Blumenschein, complainant, who Affiant believes is a credible and reliable witness, who told Your Affiant that on January 27, 2013, Ana Maria Gonzalez. Angulo, Defendant, with whom he has a casual sexual relationship with. gave him coffee to drink that had an odd taste. George Blumenschein further told Your Affiant that he.and Defendant were the only ones present at her residence and she made the coffee that she provided to him. Complainant told iant that the coffee tasted s weet and he talked to Defendant about it because he preferred his coffee black to which Defendant stated she put Sp1e.nda in it. Complainant told Affiant that Defendant later made another cup of coffee for him, but instructed him to finish the first cup first. Complainant told Affian1 that he drank both cups of coffee and they both lasted sweet. Complainant told Your Affiant that. with in.four hours of drinking the coffee, he began experiencing slurred speech, poor balance and loss of fine motor skills. Your Affiant leamed from the medical records of George Blumenschein that sixteen hours after drinking this coffee, he was admitted to a hospital emergency centerwhere he was found to have central nervous system depression, cardiopulmonary complications., and ' renal failure. Both Complainant and Defendant-work together at M,O. Anderson Cancer Center in the field of Cancer research as Mcdical Doctors. Affiant spoke to Wesley Harrott who is the Executive Director. Research Administration, Research Administration Office. M.D. Anderson Cancer Center, a credible and reliable person. who stated that ethylene glycol is present in all the labs at M.O. Anderson. Therefore. Defendant would
have access to the substance.

Your Affiant also spoke to Amit Lahoti, Associate Professor, General Internal Medicine, MO, who indicated that a microscopic urinalysis showed diffuse thin elongated crystals consistent with monohydrate calcium oxalate and that 24 hour urine contained elevated oxalic acid, which is consistent with ethylene glycol poisoning.

Or. Guale stated that a sufficicnl quantity of ethylene glycol ingestion could cause death in an adult.

Dr. Guale concurred with the assessment of Dr. Lahoti and further indicated that ethylene glycol would not have been found in blood or urine

Your Affiant also spoke to Dr. Fcssessework Guale, who is the Assistant Chief Toxicologist at the Harris County Institute ofForensic Sciences, .

Y Affiantwas provided with a Medical Toxicology Report that was prepared by Wayne R. Snodgrass. M.D, Ph.D., Professor Head, Clinical PharmacologyToxicology Unit. Medical Director, Texas Poison Center - Houston/Galveston. The Medical Toxicology Report indicated that the acute severe metabolic acidosis in George Blumenschein more likcly than not was due to ethylene glycol poisoning; the lnitial elevated anion gap but no osmal gap was consistent with his delayed presentation between the time of ingestion of an ethylene glycol-containing liquid and the time he arrived at the hospital; and the lackof detected serum ethylene glycol is consistent with his delayed presentation. This report also indicatcd that ethylene glycol has a sweet taste, which is consistent with Complainant's repon ofa sweet flavor to the coffee.
Complainant stated that he spoke with Defendant in mid-February and she stated that she hired a lawyerbecause she-had access to ethylene glycol.
is still currently under a Doctor's care due to his kidney injury from the poisoning.
Affliant leamed from Complainant' s medical records that he had a protracted loss of useof his kidneys and had to undergo dialysis for a time and
