METHODS: Loose Bowel Movement M The patient is advised to follow and take the prescribed medication regiment needed

to the fast recovery and effective treatment. Teachings and information about medicine and its side effects are also given. The following medications were prescribed as follows: - Nifuroxazide (Ercefuryl) susp. ml BID - Bacillus clausii (Erceflora) via OD E The patient is encouraged to have complete care and rest and intake of plenty fluids to replace the lost liquids in the body. T The patient is encouraged to do proper hand washing in order to prevent the passing of infectious germs. Also the patient must stay nourished and well hydrated and take medications exactly as directed by physician. H Patients with severe diarrhea must maintain adequate hydration, by mouth if possible. Also, the patients are encouraged to maintain a clean environment. O Instruct the patient to report to his physician a week after the discharge for a follow up check up. D Encourage frequent intake of soft, easily digested foods such as bananas, rice, wheat, potatoes, noodles, boiled vegetables, crackers, and soups. Encourage hydration with fruit drinks, tea, "flat" carbonated beverages, and water. Patients should avoid high-sugar drinks, caffeinated beverages, alcohol, high-fiber foods, greasy or spicy foods, and dairy products.

Transient Ischemic Attack M Advised the patient to control health conditions, High blood pressure, diabetes, and high cholesterol all increase your risk of a TIA or stroke. Take your medicine as directed. Follow your caregiver's instructions to check your blood pressure and blood sugar levels as necessary. E Advise to do deep breathing exercises, Have ambulation with assistance and support as tolerated. Perform ADLs involving hygiene and self care. Instruct the patient to comply with drug treatment regimen.

H Demonstrate to place two pillows on bed when asleep to prevent injury and other accident precautions. Limit alcohol and do not use street drugs or smoke cigarettes. Support the affected part to prevent development of pressure ulcers. Provide energy conservation techniques and adequate rest periods. O D Instruct to return a week after discharged as order by the physician Reinforce to maintain low salt, and low fat diet. Increase fluid, fruit and vegetable intake.

TAH BSO Total Abdominal Hysterectomy Bilateral Salpingo Oophorectomy M Ensure that that client will follow doctors order regarding taking medications at home. Mefenamic Acid must be given TID O.D for pain Instruct the relative to follow medication regimen E T Encourage client to exercise regularly.

Inform client to seek necessary treatment as doctors advised. Educate and instruct the family of the patient if the proper taking of medications on time and the proper wound care to avoid contamination and infection at the surgical site. Encourage ambulation for his early recovery. H O Encourage client to practice general hygiene to prevent infection.

Inform client to make necessary follow up treatment as doctors advised. Inform the family of the patient to have her check up 2 weeks after the discharge for the continuity of the treatment. D Teach client to eat balance and healthy diet to promote healing and to gain strength. His meals should include Vitamin C rich foods for wound healing. BAIAE - Bronchial Asthma and Acute Exacerbation M Patient will be compliant to take cardiomax 1cap three times a day for 10 days and Dexone 500mg two times a day for 15 days E - Patient will verbalize need importance of exercise and demonstrate proper initiation of appropriate exercise and needs to rest inbetween periods of activities T Patient will know appropriate treatment like using nebulization

H O D

The patient must take a bath daily and know the proper oral hygiene Patient must come back after 2 weeks at the OPD Low salt and low fat diet. Increase fluid intake.

Leptospirosis M Administer Paracetamol 500mg tab for pain and fever

E Upon discharge, encouraged family to provide the patient a comfortable, clean and quiet area or environment T Instructed patient to comply on medication therapy.

H Avoid contact with urine if possible, and wear protective clothing (gloves, etc.) if you need to handle urine. Practice good hygiene including careful hand washing. Disinfect surfaces where infected pets have urinated (antibacterial disinfectant or diluted bleach solution). Follow your vet's advice for treatment and make sure all medications are given as directed. O Encourage the patient to visit 3 weeks after discharge and completion of antibiotics D Encouraged patient to eat nutritious foods like vegetables and fruits rich in Vitamin C to boost immune system such as orange, papayas, strawberries, guavas, cauliflower, broccoli, etc.
