CONTRACT OF SERVICE

KNOW ALL MEN BY THIS PRESENT:

This contract made and entered by and between: Provincial Government of Sarangani, a local government unit with office address at Alabel, Sarangani Province herein represented by GOVERNOR STEVE CHIONGBIAN SOLON herein after referred to as the First party. -And-

DR. LAMBERTO LIM GABOR JR., of legal age, single, a duly licensed Physician, Filipino and with residence address at General Santos City, herein referred to as the Secondary Party.

-WITNESSETH -

1. That the first Party is in dire need of the services of the Secondary Party who shall perform work that cannot be performed by the regular personnel of the First Party. 2. That the First Party hereby employs the Secondary Party as a Reliever Physician for province run hospital of Sarangani to augment the medical services of the said hospital; 3. That the Second Party hereby possesses the education, experience and skills required to perform the job as described herein; 4. That in view hereof the Second Party is hereby contracted as Reliever Physician from the period July 01, 2013 until December 31, 2013 by each party with a commensurate rate of Pup 3,636.95 per 24 hours duty charge to Medical Services Deliver Enhancement; 5. That as a Reliever Physician, the Second Party is expected to perform the following function; a. Shall render two 24 hours duty per week in the assigned hospital. b. Clinically manage patients based from patients physical history, finding and clinical signs and symptoms; and admit or discharge patients accordingly. c. Inform patients of his rights and involve him/her and his/her family in the planning of care, and provide health teachings; d. Shall be the resident physician in-charge of all admitted patients during her tour duty and conduct regular rounds of these patients during ER, OPD and WARD duty; e. Shall answer all ward referrals and manage them accordingly; f. Shall assess/re-assess patients as needed in the observation room, the ER general wards; g. Shall assess all laboratory results of patients from the OPD, ER & WARD and manage accordingly; h. Shall endorse critically-ill patients to the incoming ROD before ending her tour of duty; i. Shall attend all clinical conferences, reporting and meetings set forth by the hospital and the Provincial Health Office; j. Shall completely and properly fill-up the patients records before forwarding him/her to the ward, upon discharge or upon declaring death to the patient in accordance to the hospitals policy on patients record; k. Shall be responsible in providing Medico-legal Certificate of patients if necessary; l. Shall be allowed to participate in medical planning and budgeting; m. Shall observe all policies and procedures set by other Departmetnsand services of the hospital; n. Directly report to the Chief of Hospital;

That it is under that this contract does not create an employer-employee relationship between the First Party and Second Party; That the services rendered hereunder are not considered and will not be accredited as government service; And that the latters is not entitled to benefits enjoyed by the regular personnel of the First Party;

7. That this contract may be terminated for cause by any of the contracting parties by giving a written notice to the other party at least one (1) month before the effectivity of the termination.
