FACILITY DATA COLLECTION STATEMENT OF WORK

Project: Facility Data Collection Services A. Background: The [insert owner name] (Owner) campus consists of approximately [insert square footage] square feet of building space and over [insert acres] acres of located at [insert address]. To manage facility maintenance, the Owner (does use or does not use a ) Computerized Maintenance Management System (CMMS) called [insert software name and modules]. Currently users, buildings, and rooms data reside in the CMMS database. Equipment data [has or has not] been entered into the CMMS database. Currently, a Preventive Maintenance Program [is or is not] implemented. Data collected as a result of this project will be used to indentify, locate, track, and maintain mechanical equipment. B. Current Conditions: There are approximately [insert number of buildings] buildings that contain mechanical equipment. All of the buildings identified in attachment A will be part of the scope of work for this project. Ages of these buildings vary from [age in years] to [age in years] and are either occupied or in use. C. Project Scope: The Contractor shall furnish all necessary equipment, personnel, and services to collect data from facility related mechanical equipment as per specifications. 1. Which Equipment: Equipment types include, but are not limited to the types indicated below in table 1.1. This does not include hoist and cranes, medical equipment, safety equipment, underground equipment, movable equipment, vehicles, and similar items. 2. How much Equipment: Data is to be obtained from all facility related maintainable mechanical equipment. 3. How to collect Data: Data is to be collected from the actual equipment, but can be supplemented by Operation and Maintenance Documentation, Owners Manuals, drawings, specifications, and other documentation. 4. What to do with the data: Data is to be imported into the CMMS software database for use in the appropriate CMMS module in order to enable the identification, location, tracking, reporting, and maintenance of inputted mechanical equipment. Maintenance will include scheduled and non-scheduled work that can be processed with a work order. 5. Where to find the equipment: The buildings that have equipment are indentified in attachment A.

D. Other tasks that may or may not also be included in the Scope are: 1. Asset Data Collection: If requested by Owner, collect Asset Data, including room numbers, assets unique name, unique facility number, , type of facility, , asset location, , building prioritization code, use code and status, and equipment identification. 2. Asset Bar Codes: If requested by Owner, provide the application of equipment barcoding of assets. The inventory shall include the collection and/ or verification of bar code data for each individual asset, , year manufactured, , serial number,, asset location,, make & model. Additional inventory requirements may be identified during task order development. 3. As-Built Drawings: If requested by Owner, correct or create as-built drawing files to reflect current as-built conditions. 4. Pictures: If requested by Owner, provide pictures of the equipment and of various maintainable items within the equipment, that is not limited to items such as filters, belts, grease fittings, and valves, are required. Pictures will be attached or embedded to the equipment description in the CMMS as per the CMMS documentation. 5. Procedures: If requested by Owner, provide the manufacturers procedures will be embedded into the CMMS to aid in maintenance of various items. Every maintainable piece of equipment inputted into the CMMS that has manufactures specifications for maintenance will have a procedure attached to the piece of equipment within the CMMS. A transcription of the procedure is allowed when a cut sheet or specification is not available. 6. BIM: If requested by Owner, provide Building Information Modeling (BIM) data. 7. Preventive Maintenance Plans: If requested by Owner, provide a Preventive Maintenance (PM) plan for each piece of equipment identified and collected. The PM plans shall be built directly into the CMMS. Prior to establishing PM plans, the contractor shall work with the Owners in an effort to standardize PM plans to the maximum extent possible. If asset maintenance records are available the contractor shall review the preventive maintenance records for each asset to determine the maintenance schedule, past history, procedures followed, and industry standard wrench time, and actual completion times. Based on this review, the Contractor shall provide recommendations

about the PM interval (is it too frequent, too infrequent?), compare the procedures to industry standards and recommend improvements. To the extent possible, all PMs across buildings for the same equipment should be standardized.

a. If asset maintenance records are not available or the asset is new, the contractor shall develop a preventive maintenance plan for the asset based on manufacturer recommendations, industry standards, code requirements, and input from the Owner. This plan shall include at a minimum applicable reference materials, a recommended maintenance schedule, typical parts required (if any), recommended maintenance procedures, applicable OSHA safety requirements, and industry standard wrench time including mobilization.
