A non-refundable administration fee of 100 must accompany this form or it cannot be processed. Please note: this fee applies to all non-University of Glasgow students. Payment will be accepted in the form of a cheque made out to the University of Glasgow as follows: 1. a sterling cheque made out to the University of Glasgow and drawn on a British bank. 2. a US dollar cheque made out to the University of Glasgow and drawn on a US bank to the equivalent of 100.00. 3. a cheque made out in Euros to the University of Glasgow and drawn on a European bank to the equivalent of 100.00. 4. The general guidelines concerning the Visiting Elective Programme in this Faculty are as follows:5. Students are permitted one elective only at this University. Students are permitted one elective in one department. Students should note that the dates of an elective can only be altered in exceptional circumstances. 6. Students must be registered in their penultimate or final year of medical school. 7. Electives are offered for a maximum of 6 weeks. 8. ACCOMMODATION CANNOT BE ARRANGED BY THIS OFFICE - advice on obtaining accommodation will be provided with your placement. A photocopy of your passport must be included with your application. This must be verified on the back as a true likeness by the Dean/Head of your University.

9. Students should note that the hospital to which they have been admitted will scrutinise documentary evidence of immunisation against Tetanus, Polio, Tuberculosis, Hepatitis B, Mumps/Measles/Rubella and Diphtheria. In the case of Hepatitis B the document should give a level of antibody measured in international units/l and should indicate whether or not the student has immunity. In the absence of immunisation against Tuberculosis, a negative Mantoux test within one year of commencement of the placement is required. In some hospitals, serological evidence of immunity to VZ is also required. Photocopies of the documents must be submitted with the application form. When you arrive, you should present yourself at the Occupational Health Department of the hospital concerned with the originals of your documents. Where applicable, an English translation of these documents should be provided. 10. Students requesting electives in an exposure prone area (i.e. surgery, ophthalmology, cardiac theatre labs, midwifery) also require evidence of Hepatitis C. DUE TO NEW HEALTH CLEARANCE GUIDELINES FROM THE SCOTTISH GOVERNMENT, OVERSEAS MEDICAL ELECTIVE STUDENTS CANNOT PERFORM EPP ACTIVITIES. THEY ARE THEREFORE HERE IN AN OBSERVATONAL CAPACITY ONLY. 11. Applicants should provide a validated report or letter from their respective countrys police stating that he/she has no criminal convictions. Please note that an original document is required. Photocopies will not be accepted. If an applicant has at any time resided in the UK a Disclosure Scotland form should also be completed. If an applicant has not resided in the UK at any time, there is no need to complete the Disclosure Scotland form. 12. Considerable public anxiety has been aroused by suggestions that doctors who are themselves suffering from AIDS or who are HIV positive might endanger their patients. The risk of a doctor transmitting the virus to a patient is regarded as extremely small. However it is imperative, both in the public interests and on ethical grounds that, before accepting an elective, you inform the supervisor allocated to you so that he/she can decide whether the particular placement offered is appropriate. When you receive an offer of an elective, please sign the form indicating that you have read and understood this statement and return it with your acceptance. 13. Electives offer no remuneration, nor is financial aid available from the Faculty. 14. It is a condition of acceptance of students for an elective that they have sufficient command of English to benefit from training in a UK University. 15.
