Loss Passport (a) Six Photographs (one to be attested) (b) Passport application form to be attested. (c) Police report about loss of passport (d) Statement of two Pakistanis. Please attached photocopies of their valid Pakistani Passport OR NICOP (e) Passport application form and Loss Passport form to be filled in (f) personal interview is necessary.
