We are not selling any type of service or product, but are conducting a statewide survey on important health issues for the University of California School of Public Health and the California Department of Health Services Office of AIDS. Your phone number was randomly chosen from all households in California. The information from this survey will be used to improve AIDS prevention activities in California. The interview will take about 20 minutes of your time. Before we begin I need to read you the following brief explanation. All of your responses are confidential and I will not ask you for your full name or address. None of the information that you provide will be linked to your phone number and there will be no way to identify anyone when the responses are analyzed. At the end of the survey I will provide you with a toll free phone number to an AIDS hotline where you can ask more questions about HIV and AIDS. Some of the questions are about things people do that put them at risk for AIDS, like sex and drug-use. Some people find these questions embarrassing; others do not. If you would like to participate in the survey but would prefer to schedule the interview for another time or at another phone number that would be more convenient for you, I can do that. If you agree to participate in the survey, we can stop the interview at any time, or we can do it in more than one session. If you do not want to answer a certain question we can skip it and go on to the next question. Although I am not going to offer you any money or other benefits for participating in this survey, I hope you will know how important your responses are in helping us to prevent the spread of HIV and AIDS.
