Terms and conditions: Cosmetic Surgery Packages 1.

INCLUDES: Before your admission (a) Pre-operative consultation with the surgeon (b) Laboratory testing: Necessary for the procedure, if required by surgeon, as CBC, PT & PTT, Urinalysis. Radiology Studies: Chest X-ray (if required) During your stay in our medical centre: Doctor Fees (c) Surgeon and anaesthetist fees Operating Room Charges: (d) Medical Equipment and Medical Supplies necessary for the procedure (e) Use Usage of facilities, consumables and drugs during operation and stay at the medical centre; OT Room, Recovery Room, OT Nurses, If required, Implants (prices vary depending on requirement) Anti-embolism stockings which may be required during the surgery, e.g. to prevent DVT (Deep Vein Thrombosis) Accommodation for standard nights in the surgical floor including: (f) Suite Room (g) Nursing Service Charges (h) Service Charges (i) Regular menu food charges for patient only. Meals during stay at the medical centre are restricted to patient at RM90/day or RM30/meal. (B,L,D) (j) Routine medication used for the procedure After your discharge: (k) Take-home-kit drugs and dressings for up to seven days after discharge and antibiotics for the prescribed period; (l) Removal of stitches, dressing or plaster if required 5 days after discharge (m) 1st post-surgery follow-up; (n) Subsequent follow-ups would require patients to pay for consumables (disposables), drugs and dressing. 2. EXCLUDES: (a) Where patients would like to extend their stay at the medical centre, extra nights stay would be chargeable at prevailing rates subject to availability of rooms. Meals are charged separately. (b) Charges supportive pressure garments required during or after surgery. (c) Important Note: All patients over 40 years old will be required pre-operative testing to ensure their safe condition for surgery. These tests are not included in the package pricing. Pre-operative studies such as laboratory tests, pathology, radiology studies, etc.; (g) Any other medical or non-medical charges, not related to the procedure/s. (h) Subsequent follow-ups would require patients to pay for consumables (disposables), drugs and dressing, medications, consultation with other specialists or doctors, diagnostic tests and investigations. (i) Items of a personal nature such as in-room soft drinks, telephone charges and guest meals will be charged at the normal rates in effect and are subject to service charges and taxes if any. (j) Charges for intensive care, if necessary. (k) Meals ordered by guests of patient charged based on invoices/bills and are subjected to service charges and taxes if any (l) Any cost not specified as included Patient Eligibility 1. The package is applicable for elective, uncomplicated patients. Patients with the following conditions are considered to be complex or high risk cases and will not qualify for the package: Bleeding Disorders Patients with underlying severe medical problems such as HIV and uncontrolled diabetes Not followed pre-operative instructions 2. If the surgeon determines a patient as a complicated case or the patient has a body mass index of more than 40 (BMI >40) or weighs more than 100 Kilograms, there will be a surcharge of 20% imposed on the normal hospital procedure prices.

Admission 3. Beverly Wilshire Medical Centre Snd Bhd reserves the right to refuse the admission of a patient for any reason. 4. We aim to offer all patients their first choice of admission date, however we reserve the right to determine the date of admission of the patient and, if necessary, change the date of admission. Prior notice will be given, whenever possible, should the admission date need to be changed. 5. Costs incurred that are not included in the package price will be charged at the hospitals standard rat es. 6. A RM1,000 deposit will be collected from the patient upon admission for surgery to cover for any incidental expenses that may be incurred during surgery and ward stay. This will be refunded in full when you are discharged, if no additional costs were incurred during your stay. Discharge 7. The decision as to whether you are fit for discharge rests with your surgeon. Should you wish to stay in hospital after you have been declared fit for discharge, the hospitals standard charges will apply and will be billed for any additional costs. 8. If you discharge yourself against the advice of your consultant no further services will be provided as part of the package. 9. No refund will be given if you leave the hospital earlier than expected. Complications 10. The package price covers the cost for uncomplicated standard procedures only, if any related physical conditions or complications become apparent pre-operation, post operation or during surgery, the standard hospital and doctor charges will apply. 11. Any revision surgery will be chargeable at the prevailing rates by the hospital and consultants, each patients case will be reviewed on a case by case basis. Written estimation 12. The surgeon and their secretarial staff only have authority from the hospital to quote for hospital charges contained within the latest copy of the hospital price list issued to them. Any price mentioned by them outside the price list is subject to written confirmation by the hospital. The hospital will provide the final quotation to the client based on the surgeons recommendations. 13. The quoted price for the package is valid for a period of 30 days from the date of issue. A booking for the surgery must be made within this period. 14. The quotation is subject to change depending on the results of any pre-operative assessment and recommendations made at the consultation with the surgeon. The hospital reserves the right to withdraw the quoted price at any time prior to surgery. Surgeons fees 15. All surgeons are self-employed and provide their services direct to the patient. 16. The surgeons fees for the operation and a follow -up consultation are included in the quoted price and the hospital acts as their agent in the collection of them. Surgery booking deposit 17. To confirm your surgerys theatre booking, the hospital will require a minimum of a 30% deposit of your (Total Price) a greed for your procedure/s. This should be paid a minimum of three (3) weeks in advanced, of your surgery date. 18. If you cancel or postponed your surgery within two (2) weeks of your agreed and booked surgery date, an administrative charge of 5% of the total agreed surgical procedure price or up to a maximum of RM1,500 will be imposed on the deposit received, if surgery is cancelled or postponed without reasonable explanation. If you give more than 2 weeks notice of your surgery date to cancel your surgery , the operation theatre will be released and you will be refunded for the amount paid in advance, minus any bank charges, incurred by the medical centre. Payment 19. Payment must be made in full prior to admission. The hospital reserves the right to refuse admission to a patient who has not paid in full, which may result in your surgery to be postponed or cancelled. For Malaysian customers only: Personal cheques are accepted by prior arrangement only, and cheques must be received 4 weeks before your surgery date. Allow an extra 5 days for the cheque to clear. Bank transfers for local patients can be made to the bank account details below. Australian clients can make telegraphic transfers in Malaysian Ringgits through Ozforex http://www.ozforex.com.au/ or your bank: New Zealand patients can go to NZforex http://www.nzforex.co.nz/ or their local bank. Most clients find that the transfer rates with Forex are more economical.
