1. What changes or new benefits plans are included in the Mattress Firm benefits programs? 2. Who is eligible to participate in Open Enrollment? Open Enrollment is available to Associates who are scheduled to work at least 30 hours per week and who have also attained 60 days of continuous service. Associates scheduled to work less than 30 hours per week and Temporary Associates are ineligible to participate in Mattress Firm benefit plans. During the period from September 7, 2013 September 22, 2013 eligible employees can either enroll or make changes to their current benefits selections.

3. If I dont want benefits, do I still have to enroll? Participation in the benefits plans is on a voluntary basis. However, you may cancel your Medical, Dental, Vision, Voluntary Term Life, Voluntary Accidental Death and Dismemberment or Flexible Spending Account during Open Enrollment through UltiPro if you no longer want coverage. 4. How can I learn more about the new Medical High Deductible Plan and Health Savings Account? Information for both plans is located on the Benefits Web Page at www.mattressfirmbenefits.com 5. What are the new rates for 2013 2014 benefits plans? Rate information for each plan is located on the Benefits Web Page at www.mattressfirmbenefits.com 6. If I dont want any changes to my current benefits, do I have to do anything? If changes are not required to your current Benefits, you do not have to do anything at this time with the exception of Flexible Spending Account, Voluntary Term Life and Voluntary Accidental Death and Dismemberment. You must re-enroll in Flexible Spending Account each year. Also, this year due to a change in our HRIS, you must re-enroll in the Voluntary Term Life and Voluntary Accidental Death and Dismemberment plans if you want to continue coverage. 7. How can I enroll in the Benefits Plans? Enrollment in the benefits plans is processed through Ultipro which can be accessed through the Associate Resources Page of the BedPost. You can also access the website from your home computer at https://n11.ultipro.com. An Ultipro Navigation Guide,

8. When is the deadline for Open Enrollment? The deadline to complete your enrollment is Sunday, September 22, 2013. 9. Who is considered an eligible dependent? Eligible dependents include: Spouse (not fianc, but your legal spouse as determined by applicable law), your child (by birth or adoption) or your stepchild (if still married to that childs parent) up to age 26, legal dependent (proof required), QMCSOs (legal documents required). You must include the social security number, date of birth, gender and relationship to you for each dependent in order to complete the enrollment. If you are enrolling a dependent for the first time, a copy of the birth certificate, marriage certificate or any legal documentation must be faxed to Benefits at 1-866-593-1124 in order to verify dependent eligibility. 10. What if I enroll an ineligible dependent in the Benefits plans? It is very important that only eligible dependents are enrolled. Ineligible dependents could cost the plan thousands of dollars every year and increase the cost of benefits for all of our employees. Employees will be held responsible for paying back the claims on any ineligible dependents, and may face disciplinary action.

11. What should I do if I cant log into UltiPro? The most common situations which prevent logging into UltiPro are: a. Forgotten password simply email Ultipro@mattressfirm.com and it will be reset within 48 hours. b. Wrong user ID If you have not logged into the website before and you have a common last name (i.e. Smith), the standard user name may already be in use (i.e. Smith J). So your user ID may actually be Last Name, First Initial and a number. Email Ultipro@mattressfirm.com if you think this might be your problem. 12. Will there be any webinars where I can ask questions and learn more? Presentations covering each on the benefits plans are available on the Benefits Web Page at www.mattressfirmbenefits.com . They can be viewed at your convenience via your PC or Mobile Device. 13. How can I learn more about the cost and plan changes to my Benefits? The 2013 - 2014 Benefits Enrollment Guide contains the new rates, contact information and other valuable information regarding the Benefits plans. It is located on the Benefits Web Page at www.mattressfirmbenefits.com under the Open Enrollment tab. When will my Benefits become effective? New benefits or changes to your current benefits will be effective October 1, 2013. 14. When will the new deduction amounts be reflected on my paycheck? New deductions will be reflected on the October 15, 2013 paycheck for semi-monthly paid associates; and on the October 18, 2013 paycheck for bi-weekly paid associates. 15. When will I receive my new benefit cards? If you havent made any changes you will continue to use your current card. If you are newly enrolled, or have added dependents to your benefits, your new benefits cards will be mailed by the first of October. Effective October 1, 2013, you can print a temporary card from the Benefits Web Page at www.mattressfirmbenefits.com under Temporary ID Card. 16. Do I have to enroll in one of the Medical Plans in order to participate in any other plans? Participation in the Medical Plan is not a requirement for the other plans, with the exception of the Health Savings Account. If you want to participate in a Health Savings Account, you must also enroll in the Medical High Deductible Plan. 17. When can I enroll in the 401(k)? Enrollment in the 401(k) can occur at any time throughout the year, once you meet the eligibility requirements. In addition to the eligibility requirements described in Question 2, you must also be at least 20 years of age to participate in the 401(k). You can enroll online at www.mattressfirm401k.com or call OneAmerica directly at 1-800-249-6269. The Plan Number for Mattress Firm is G36192.
