SPIRITUALITY ASSESSMENT SCALE SURVEY Thank you for taking the time to participate in this attitude survey.

It should not take more than 10 or 15 minutes to complete. All your responses will be confidential; there is no way to identify individual surveys. Please be as honest as possible in your replies. Each of the 30 statements listed below describes a feeling, behavior or attitude. Indicate your level of agreement with the statement by ranking it according to a seven-point scale in which 1 is strongly disagree with the statement and 7 is strongly agree with the statement. Indicate your response by entering one number between 1 and 7 in the blank space to the left of the item. Sample Response Ranking: 1 = Strongly Disagree/Strongly Does Not Apply to Me 7 = Strongly Agree/Strongly Applies to Me Sample Item: 1. 7 I read part of a newspaper every day. By entering 7 in the blank space provided to the left of the statement, you are indicating that the statement strongly applies to you.

It is important that you respond to all the statements if possible. Please do not use the middle ranking (number 4) as an I dont know category. For purposes of this survey, the term transcendent means that which is beyond and independent of the material universe. Other terms that may be used to refer to the same concept are God, divine, or spiritual. Do not labor over any statement or analyze it deeply. Put down the first answer that comes to mind, and go on to the next one. This is not an examination. There are no right or wrong answers. This survey is simply a study of peoples attitudes. Please indicate your own responses by the numbers you enter.

I help other without thinking about getting rewarded. Before making an important decision, I normally pray or meditate. Prayer or meditation has the power to change my life Most of the time, I present a false front of who I am. Transcendent influences do not impact me very much. I will help others even when it requires a sacrifice. Prayer or meditation doesnt make much difference in life. Prayer or meditation is low on my list of things to do. I have experienced the divine in my life I dont look for opportunities to be of service. I believe that spiritual guidance is available through prayer or meditation. I would rather fail at a task than ask for help. I give credit to others for their good ideas. Its delusion to think that prayer or meditation is effective. I accept responsibility for my wrongs and make amends even when I pay a price for doing so. I dont worry about telling lies if they dont hurt others. I dont pray or meditate at all. I prefer to do good deeds anonymously whenever possible. I give more than is asked of me most of the time. Material success is more important to the good life than spiritual growth. I attempt to practice spiritual values in all areas of my life. I dont blame others when its my fault. When I have reached the limits of my capabilities, I ask others for help without shame ore embarrassment. Being the center of attention is desirable. I pray for others or meditate on their behalf.
