APPLICANT'S UNDERTAKING/WAIVER
1. I/We acknowledge, as personal indebtedness to GSIS, whatever amount is approved by way of housing
Loan and hereby agree to pay such loan in accordance with the terms thereof.
2. I/We shall comply with all loan requirements and execute all supporting papers and/or documents to the
satisfaction of the GSIS.
3. I/We agree to update/liquidate any other GSIS loan arrearages prior to approval of this application in
compliance with the Claims and Loans Interdependency Policy (CLIP) of the GSIS.
4. I/We agree that the loan facility shall automatically be cancelled, if not availed within six (6) calendar
months following the month of the notice of approval of loan.
5. I/We agree that the proceeds of the loan shall be used solely for the purpose applied for.
6. I/We agree that the GSIS shall retain the copy of the title of the property offered as collateral during the
existence of the mortgage.
7. I/We shall conform to such changes or modifications as may be made by the System on the amount of
loan and/or terms thereof.
8. I/We agree that the construction or improvement of the house shall be completed not later than six (6)
calendar months after first release of the loan, unless extended for justifiable cause, otherwise the
unavailed portion shall be deemed waived after said period.
9. I/We agree that the total amount of loan releases shall be charged with interim interest at the rate
applicable to the approved loan to be deducted from the proceeds of subsequent releases. Upon final/full
release of the loan, the total releases shall be charged with interest at the same rate applicable to the
approved loan, accruing from the day of final/full release up to the last day of the month the release was
made.
10. I/We agree that the property offered as collateral shall be adequately insured against fire and/or other
perils as may be required by the System.
11. I/We shall conform with the Mortgage Redemption Insurance as may be required by the System.
12. I/We agree that the principal loan, interest and other charges due shall be payable in monthly amortization
to be deducted from my payroll or through post-dated checks whichever is applicable. I/We hereby
authorize the Treasurer/Disbursing/Collection Officer of my present or future government office to deduct
from my salary the required monthly amortization.
13. Prior to the regular deduction of the monthly amortization effected by my/our office, I/we agree to pay
directly to GSIS the required amortization to avoid penalty and surcharges in my/our account. Further,
I/We agree to pay directly to the GSIS the housing loan account, including penalties, surcharges and
interest in order to update my account in the event that the Treasurer/Disbursing or Collection Officer of
my present or future government office fails to deduct and/or remit to the GSIS the monthly amortization on
the due date.
14. I/We agree that the first monthly amortization on the loan shall commence on the month following the
full/final release of the loan or closure of the loan facility as the case may be.
15. I/We agree to issue post-dated checks equivalent to my monthly amortization for the remaining term of the
loan in the event of my/our separation/termination from the government service while still having an
outstanding balance in the housing loan.

I/We recognize that the GSIS will only assist us in the financing requirements for the acquisition,
construction or improvement of our housing units, hence, the GSIS has no participation whatsoever in the
development of the subdivision and construction/improvement of our housing units. As such, I/we for
ourselves and our heirs and successors in interest, hereby forever release, waive and forever discharge
the GSIS, its successors and assignees from all actions or courses of actions, claims and/or demands
pertaining to whatever defects or deficiencies in the construction/repair/improvement of the house subject
of this loan application or defects in the development of the subdivision where the same housing unit is
located.
17. I/We agree that any misrepresentation of a material fact is a ground for disapproval of this loan application
or for the GSIS to declare the mortgage loan due and demandable, whether partially or fully released.

THE FOREGOING STATEMENTS MADE FOR THE PURPOSE OF OBTAINING A HOUSING LOAN ARE
EXPLAINED TO MY/OUR SATISFACTION AND UNDERSTANDING, HENCE, I/WE AGREE THAT THE SAME
SHALL BE CONSIDERED AS PART OF THE MORTGAGE CONTRACT.
