SIR CHARLES GAIRDNER HOSPITAL

Welcome to the Outpatient Department of Sir Charles Gairdner Hospital (SCGH). The department provides a consultative service for patients referred by general practitioners and specialists from inside and outside the Hospital.

Your appointment

Please present your appointment card to the receptionist upon your arrival at the clinic. Advise the receptionist if you have changed your address, if the appointment is related to workers compensation or a motor vehicle accident or if you are an overseas visitor. Allow at least two hours for your appointment. We recommend you do not make any other plans during this time. Every effort will be made to see you in a timely manner. After your appointment, hand all appointment cards and any request forms to the receptionist for processing before you leave.

Pharmacy

Where to go

The Hospital Pharmacy is open from 8.30am to 5pm Monday to Friday and is located in the Outpatients Department, ground floor E Block. A private pharmacy is located on ground floor E Block in the retail area.

The following clinics are located on the ground floor and first floor of E Block: ENT (Ear, Nose and Throat) Eyes Orthopaedic Dermatology Biopsy Diabetes Centre Neurology Department Other specialist clinics, X-ray, specimen collection/pathology, a cashier and the patient appliance centre are also located in E Block.

Accomodation

Limited on-site accommodation is available for patients from country areas. For details, telephone the Welfare Assistant on 9346 4666. Patients receiving treatment for cancer may wish to stay at the Cancer Councils AH Crawford Lodge. For details, telephone 13 11 20.

What to bring

Travelling to Hospital

We encourage patients to consider alternatives to driving themselves to Hospital where possible. Car set down and pick-up zones are located outside the main entrance to G Block. Buses stops are located along Hospital Avenue. For bus information phone TransPerth 13 62 13 or visit the website www.transperth.wa.gov.au Taxis are available at the rank on Hospital Avenue opposite G Block. Free call taxi phones are available in E and G Block reception. Charlies Chariot is a courtesy buggy for those in need, collecting passengers from car parks 1 and 5 regularly from 9am to 4pm weekdays. Call 0429 996 822 or ext 4645. Car parking is limited. Visitor pay parking is located along Hospital Avenue and off Monoash Avenue, Verdun Street and Gairdner Drive. There are limited ACROD parking bays (see map).

If applicable, make sure you bring the following to your Outpatient appointment: Private x-rays Private laboratory reports LIst of current medication Spectacles Hearing aids Medicare card Repatriation benefits card Any concession cards

If you have difficulty speaking English, we can arrange a professional interpreter for you.

Wheelchairs

A wheelchair can be requested at the reception desk, E Block main entrance.

No Smoking

Smoking is not permitted anywhere inside the Hospital or outdoors on Medical Centre grounds.

Cancellation
If you are unable to attend your appointment please cancel as soon as possible by calling 1300 855 275. Notifying us enables us to offer the appointment to someone else.
