Introduction
a) Background Of the Project
Many primary care clinicians work to deliver the most appropriate type of care and avoid unnecessary and expensive hospital admissions. Now days, we can easily see that our life is so much busy. In the modern life in a care centre or in a hospital, there is no time for entering details manually because of the changing life style and fast approach to each work. In developed countries, mostly the people do work using computers for time savage to fill entries and to maintain records. Because its time saving and you can easily find the records in a short span of time. he present system lacks as same data gets repeated over and over, data is stored in files and folders so it is freely available to anyone. here may occur inconsistency of data. here exist repetition of work so if there are any changes to be made, the data will have to be entered again and at time the worker would forget to make the changes. It is lot of space consuming procedure. here is slow retrieval of data. !atient "are "entre #!""$ frees up valuable office storage space by offering a simple and cost%effective means of filing, searching and rapidly retrieving patient documentation.

b) Importance Of Project

Computerized System& Manually system changes into computeri'ed (ystem that is lengthy )iles are difficult to maintain and it is difficult to relocate the existing information from the file. (o, we have no need to maintain registers and files.

riend!y user interface& *octors and users need not to learn the special skills. Its menu driven and naive users can use it efficiently without using the concept using to make the software.

"ime sa#ing and Cost $ffecti#e& It will help thousands of doctors to save time and money on office procedures and documentation. !rimary "are doctors are also able to see more patients when implemented. .

%eports& +e can maintain ,ueries, tables and data report as per demanded by the pro-ect.

&i#e faci!ity of different type of en'uiries & .istory is maintained whenever patient visit.

ormatted (ata& /very detail is maintained in their respective fields and every detail is connected with respect to table that is maintained in database.

)e* +pdation& 0ny kind of new drug or disease can be easily introduced in the software.

*atas are easily accessible with a set of permission mapping. *ecrease !aper +ork.

c) Objecti#es Of Project
!"" offers a complete and comprehensive management software application to primary care allowing practices to log and track detailed clinical information and diagnosis treatment through to prescription and electronic pathology results while automatically coding all data entered. he ob-ectives are listed into ma-or ob-ectives and the applications. he various ma-or pro-ect ob-ectives of the pro-ect are listed as& "o design soft*are& It allows fast registration, triage and patient referral to the most appropriate service within the hospital. By interoperating with system such as !"", a patient can, where necessary, be handed over electronically from !"" to a hospital system, thus ensuring consistency of care and accurate measurement and reporting of the patient -ourney. +ser interface& Its having a good user%interface. No one needs to learn the special skills and knowledge. Its menu driven and naive users can use it efficiently without using the concept using to make the software.

Business ,ogic- his pro-ect consists of various logics which are applied in businesses in organisations. hese logics handle the information exchange between the database and user%interface. hey detail the flow of information and data of patients in the hospitals.

(atabase (esign& *atabase is designed for the physical storage of patient details which is the efficient ob-ective of this pro-ect. It provides access to up%to%date and accurate information. hus it reduces data redundancy.

he main applications ob-ectives of the !"" are& "o pro#ide the fastest care& +hen patient first visit, he doesnt need to undergo the process of filling form or creating a file. !"" generates a uni,ue patient id by entering -ust a set few of mandatory basic fields. 0nd patient can immediately have the primary care. (istributi#e system& It acts as distributive system as all the doctors in various departments can access the basic information. "o make an efficient system & he ability to treat patients faster, keep more accurate

records and provide better office processes are a few of the many benefits that internal medicine offices can experienced while using this software. Stream!ines office procedures. here is 1no time2 spent searching for lost records because charting is written at the point of care and organi'ed right away. "harts, files and folders are all eliminated. here is nothing to misplace. 3ffice may also share patient information among staff to complete multiple tasks at once. $asy e!ectronic documentation& Many primary care doctors have also found that point% of%care charting improves documentation overall. aci!itates risk management- !atient data is securely stored and because records are automated, the risk of documentation audits is significantly reduced.

"o maintain a patient focus in office. !atient records are instantly accessible and complete patient medical records can be viewed at a glance. he !rimary "are system also captures all patient%related messages and charts them to the appropriate record.

.ppointments& +ith the help of !"", It is easy to make an appointment 4and get an appointment as early as the same day which is helpful as you don5t have to wait with an appointment and can still be seen for immediate care.

PCC soft*are is easy to imp!ement and use/ so doctors and their staff can focus on patient care 6 not on installing, learning and maintaining a complicated system. It is designed to work the way real practices work, supporting normal workflow and time%honoured standard procedures.

he planning idea is a need to handle all the work like database, tables, design, forms, business logic implementation, development #coding$, testing.
