PROCEDURE CHECKLIST ORAL CARE

PROCEDURE 1. Assess clients ability to brush her/his teeth 2. Gather Equipment. Place on an over bed table or patients side table. 3. Do handwashing. 4. Provide privacy for the patient. 5. Position patient in semi-fowlers position. 6. Place a towel under the patients chin, tucking it behind the shoulders. 7. Let patient sip water & gurgle. 8. Hold the toothbrush over the emesis basin, pour a small amount of water over it and apply toothpaste. 9. If patient is able to brush his/her teeth. Let the patient do the following: a. Place toothbrush at 45 degrees angle to gumline & brush from gum line to crown of each tooth. Brush back and forth across biting surfaces of each tooth. b. Brush tongue gently with toothbrush. c. Allow the client rinse mouth with several sips of water and spit onto emesis basin. d. Offer mouth wash or dental floss if available. e. Help the client wipe the mouth. 10. Remove equipment used. Do after care. 11. Place the patient in comfortable position. 12. Do handwashing 13. Document the procedure. Record Time procedure is done, findings related to condition of mouth & gums.
