7.1. A properly accomplished application form of the U.P. College of Medicine. Application forms will be made available upon CASH Payment of an application fee which is NON-REFUNDABLE. Checks are not acceptable. 7.1.1. P 1,000.00 for Filipino applicants 7.1.2. $ 200.00 for foreign applicants Note: The Application Form is valid only for the Academic Year applied for. 7.2. Original/Official Transcript of Records for at least the first three and one-half (3-1/2) years of Baccalaureate Program. 7.3. Four copies of 2x2 pictures signed by the applicant on the front of the picture. 7.4. Certified true copy of NSO Birth Certificate. (A photocopy is acceptable provided the original is shown for verification). 7.5. Original Certificate of naturalization for naturalized Filipino citizens. ONLY original certificates of naturalization and other documents issued by the Commission on Immigration and Deportation shall be acceptable. Applicants with PENDING naturalization papers and documents are NOT eligible for admission. 7.6. Latest True copy of Income Tax Return (ITR) of both parents. DOST Clearance for students who have availed of DOST scholarship in college. Submit all requirements in a long brown envelope with your printed name (surname first) written in pencil only. 8. The Admissions Committee and the College of Medicine reserve the right to refuse admission to any applicant on the basis of standards and criteria set forth by the Committee as mandated by the proper University officials. 9. Certificate of naturalization for naturalized Filipinos. 3. Two (2) signed most recent colored 2x2 photographs. 4. Certified true copy of transcript of records ( 3 1/2 years for those graduating in March. 5. Photocopy of diploma/s of parent/s if UST graduate/s. 6. Photocopy of NMAT score. 7. Three (3) Certificates of Good Moral Character (one each from the Academic Dean, Office of Students Affairs and a Professor). 8. Properly accomplished application form. 9. 1. Applicant must be a graduate/holder of A.B./B.S. Degree (a 4- year or a 5-year course); 2. Applicant must have taken the National Medical Admission Test (NMAT) and have obtained a score above the percentile cut-off set by CHED as recommended by the Technical Panel for Medical Education on a yearly basis. If the Applicant satisfies the above requirements, the Applicant may now: Proceed to the Office of the Registrar for Application Requirements; Pay the non-refundable Application and Processing Fees ofPhp 1000.00 at the Hospitals Cashier located at the Ground Floor, Hospital Bldg.; Present Official Receipt to the Registrar and secure Application Form and list of requirements; LIST OF REQUIREMENTS: a. Official Transcript of Records b. Original Certificate of Graduation or Candidacy for Graduation, or photo- copy of AB/BS Diploma c. Original Certificate of Good Moral Character from two (2) former professors

APPLICATION PROCEDURE The Applicant shall: 1. Submit the Application Form together with the required documents to the Registrar's Office. 2. Pay the processing fee in the Accounting Office. 3. Take the English Proficiency Test (for Applicants whose medium of instruction is not English) 4. Applicants qualified for Interview will be notified of their schedule.
