Malandag, Municipality of Malungon, Province of Sarangani after being sworn to in accordance with law, hereby depose and say that; I am legally married to Beverly T. Ocer dated May 28, 1990 at United church of Christ of the Philippines, Prk. Bayanihan, Brgy. San Isidro, City of General Santos, as evidenced by the Marriage Contract as Annex A; Our Marriage was blessed with two children namely Leysly Shine Manalo and Kim Edverly Pearl Manalo, 23 and 10 yrs of age respectively; Our Marriage became sacred and I decided to leave and have my own family. We have been separated-in-fact since early of 2007; I do promise to faithfully support the aforementioned children of mine from clothing, shelter, education, food and the like. And such support shall be given to them in person and by month; I execute this affidavit to attest the veracity and truthfulness of the foregoing statements and for whatever legal purposes it may deem serve ;
