This is an example of a student academic plan that we use to advise students. I have been using these since 2008, updating them each year as the curriculum changes. I have posted student advising hours and also accommodate those students who are not available during my regular hours. The student academic plans have been created for each Design emphasis/concentration, for each degree. Every student has this form customized with their grades. I always give a student an updated hard copy of this record at the end of our appointment. This paper system is now being replaced with Ivy Advising. A student will be able to check any time to see what he/she still needs and what has been completed.
