Birmingham: Located on the UAB Campus on University Blvd. between 13th and 14th Street South. Huntsville: Take I 65 South. In Birmingham, take Exit # 259B, 4th Ave South. Turn right on 13th Street. Look for tents in the park. Montgomery: Take I 65 North. In Birmingham, take Exit #259, University Blvd. Go straight on University approx. 0.3 miles. UAB Mini Park is on University Blvd. between 13th and 14th Street South. Sylacauga: Take 280 West. In Birmingham, take the 8th Avenue South Exit. Go right at the first light as you exit the expressway. (You will be heading toward UAB Hospital.) Go straight down University Blvd. for about 1 mile. UAB Mini Park is on University Blvd. between 13th and 14th Street South. Tuscaloosa: Take I 20 East. In Birmingham, take I 65 South to Exit # 259B, 4th Ave South. Turn right on 13th Street. UAB Mini Park is on University Blvd. between 13th and 14th Street South. Parking: Available in the parking deck on 13th Street

Schedule of Events
11 a.m. 2:00 p.m. Transplant Picnic Registration: Free meal tickets will be available for everyone who pre-registers. Registration forms should be returned to the address on the Registration Form by April 18th. Gratitude Boxes: Boxes will be available in the registration area for you to drop your doctor, nurse, social worker or transplant coordinator a note and/or picture. There are special gratitude boxes for donor family letters. Photos: Group and candid photos will be available for purchase after the event through the AOC web site, www.alabamaorgancenter.org. 11:15 a.m. Celebrating Transplantation A short program will feature speakers from the UAB Organ Transplant Program and the Alabama Organ Center. Donor Recognition Ceremony Those who have made possible the precious gift of life will be honored at a ceremony. The quilt pinning will take place as part of the Donor Recognition Ceremony. Group Pictures Group pictures of recipients and donor families will be taken immediately after the program. The location of the pictures will be announced at the program. Noon2:00 p.m. Enjoy Lunch and the Festivities! Fun for kids and adults. Writing Recipients and Donor Families For guidelines on how to write your donor family or recipients, please visit the Alabama Organ Center website at www.alabamaorgancenter.org. Click on Resources and then click on Donor Family Program for more information or call Carrie Ellis at 1-800-252-3677. Letters can be dropped off at the Donor Family tent or in the boxes at registration. Quilt Square Presentations During the Donor Recognition Ceremony, donor families will be invited to present quilt squares they have made in memory of their loved one. This year we are asking YOU to submit designs for our T-shirts. Send your design on an 8 1/2 X 11 piece of paper along with this registration form. Deadline for submissions is March 19th. All submissions become the property of the Alabama Organ Center, may be altered and by submitting you are giving permission for your design to be used by the AOC on future projects. The selected artist(s) will be recognized at the picnic.

Please register using only one name. This will help us find your registration form and free meal tickets on the day of the picnic.
