US Social Security Administration

Office of the Inspector General

Washington, DC

23 August, 2013

Sir or Madam,

My name is Jennifer Fair. I'm a disability recipient, and must write to you to report a very egregious violation of my privacy
that's gone unresolved for an unacceptably protracted amount of time.

I've tried to report this matter over the past several months to the SSA Office in Miami, Florida (Little Havana) but numerous
messages to senior officials have gone without response. This includes Miami Little Havana Branch Supervisor Doris
Fahardo (or Fajardo), Little Havana Office Manager Queen Mohammed, and most recently Miami District Manager Bernardo
Del Riesgo. None bothered to address this, despite expressing this was a serious breach of privacy issue I needed to
discuss and resolve.

To put this in some brief context, this matter goes back to late 2011 when I returned from abroad and sought to sort out my
long-time benefits going forward. Unfortunately, those relations to which I left a lot in trust (namely father and aunt) violated
that trust greatly, and I had to reestablish receiving my benefits. Despite financial and personal difficulty, the Little Havana
SSA Office sorted out the immediate issues, including having my records sent from the Melbourne, Florida SSA Office, as
I sought to establish myself in Miami. Also, to formally establish myself as sole Payee (I being Beneficiary) to which my father,
Thomas Fair, had been for me since the beginning.

To that, I made it clear to those in Miami that he and other relations had continually been and would potentially continue to be
very problematic, and that I wished my privacy to be understood and secure because they play NO part in my life nor in my
SSA affairs. Further, that nothing was to be disclosed in any way whatsoever to anyone outside of SSA especially to him or
other "relations". I put that in some written form, but mostly verbal. These so-called relations are all basically "persona not
grata" to me, and have caused me far more grief than I need or wish to express here. As Payee, he made quite a mess of
my affairs in failing to contact me about matters and in dealing with Social Security issues. I was prepared and capable of
taking responsibility, and it was also personally necessary to assume full control from his narcissistic and often abusive
nature as well.

From everything I arranged in 2011, I understood all of my records were received from Melbourne SSA. This goes to my
complaint presently, because as far as I was concerned this established formally that all business and connection with that
office was concluded. They were difficult enough to deal with, and there was other interference. Therefore, nobody at all at
the Melbourne SSA should have any need whatsoever interfering in my business and life in Miami, nor pulling my records or
contact info. I consider that enough of an affront, but I fear it went beyond.

I sought to maintain my privacy until matters stabilized and was settled. Of course, SSA could always reach me, and I'd online
access to my information as well. This past January, I sought to update my online account and request a new card because I
was very dissatisfied with the incompetent telephone assistance. On my visit to the Little Havana Office in February I reiterated
my privacy concerns in updating my contact details.

However, a month later I also received an official envelope posted from the Melbourne SSA Office. This is the reason for my
complaint. I'd no contact since 2011, but it was hand-addressed to me which they hadn't reason or need to know. The contents
weren't official or familiar, but the source was and it was alarmingly clear to to me someone at Melbourne SSA either gave my
details to unauthorized people as means for harassment or was complicit in sending unsolicited info via official means (also
surely a postal violation). Any of which is extremely dubious.

To that, It's extremely concerning and unacceptable that my information was exposed in such a manner or at all. To what extent
I still don't know, since no inquiry has been made and I've not received proper local-level cooperation to get the answers and
accountability against the Melbourne SSA officials responsible. As such, I've sought legal advice to protect my interests and to
be sure I will, especially if you cannot or won't address this. This is not an open-ended overture to you, so I ask your prompt
reply and cooperation, or I'll have no choice but to proceed and take the SSA to task for this.

Thanks for your time. My full contact details are noted below, but as it's prudent to omit my Social Security Number for security,
I'll provide for reference only on direct response to a specific official. My full name is Jennifer Amy Fair (Jennifer A. Fair) and
hope that will suffice to make a proper inquiry.

Regards,

Jennifer Fair
