Kelly is a 30 year old female client who has unwanted hair on her upper lip and also wants IPL treatments for hair reduction on her bikini line. She has had one treatment on the upper lip and has some fine regrowth. She mentioned that she expected that all the hair would be removed after the first treatment. In addition she has an active cold sore on her chin. She has decided to have all the hair in her pubic area removed with IPL and wants you to treat it today as she is going on holiday with her fianc tomorrow. She is Fitzpatrick Skin Type 2 and her unwanted hair is brown. 1. Describe in some detail the important points you would cover in your consultation with her. It is very important to explain all factors that affect client regarding IPL and Laser. Female clients we must ask about if they are taking any Oral Contraceptives, HTR, Pregnant or planning to become pregnant and the regular menstruations. Find out what method of hair removal used to date and its frequency and if there has been any reactions. Best to know if the client is currently taking any medication to find out if they are photo sensitive. Ask about their lifestyle and occupation to find out about sun exposure What is their genetic background? What previous treatments they have tried? Client expectation? Explain about possibility of Hyper pigmentation if they client has exposure to sunlight before, during or after the treatment. Explain the hair growth cycle and how laser and IPL works Explain how light works with Hair, Skin and Melanin. Explain about patch test before the first treatment. Your patch test is also vitally important as it allows us to test your skin for any sensitivity from the laser and also will ensure you are a suitable candidate for treatment Explain that the client must use eye protection during the treatment Explain that due to the nature of the treatment an exact result cannot be predicted. No guarantees have been made as to the results can be achieved. The treatment may not result in permanent hair removal. Explain about photographs taken before and after treatment on for the purpose of monitoring response to therapy. Its very important to explain to the client that its best to have a series of treatments to achieve best possible results and due to the fact that she is traveling its best for her to wait after she comes back and have the treatments. Explain the factors that affect their treatment outcome are the following: Tanning and having to leave long gaps in-between sessions, these gaps are necessary to allow tan to fade so laser will not pigment the skin.

Your heat sensitivity, the level of temperature that you can tolerate starting off, if you are sensitive to heat all that means is that you may need a few extra sessions to gradually build up the heat each time. Any changes in medical circumstances for example if you became pregnant we would just postpone treatment until you had your baby and resume treatment a couple of months after. If you wax in between your sessions this will interfere with growth cycles and may mean you will need more sessions

2. Outline a series of treatments you would recommend and the likely outcomes. Outline the pre and after care treatment you would recommend to this client PRE TREATMENT Its very important avoid direct exposure to sunlight, solariums or self-tanning products for at least 4 weeks before treatment. Avoid exposure completely during treatment and for about 4 weeks after the last treatment. Do not use products or medications that can cause skin sensitivity for example antibiotics. You must not use them for two weeks prior to treatment, but must consult your doctor before stopping. Make sure not to aggravate the skin by exfoliating, applying creams or perfume or expose the areas to heat or sun. You can cut or shave the hair between treatments. Do not wax or tweeze the hair. Hair should be1-3mm in length at the time of the treatment.

POST TREATMENT Do not have hot shower for 24 hours after each treatment Apply cool, clean and damp compresses to the area to relieve swelling Use the healing or soothing products recommended by the therapist. Do not rub the treatment area. Do not do heavy, sweaty work out for 24 hours after the treatment. Do not use saunas, spas, pools or chlorinated water Always wear clothing and 30+ SPF to protect the treated area from sun exposure and make sure to reapply the SPF 30+ as recommended. Mild erythema (reddening of the skin) can last from a few hours to few days. Pigmented areas may become darker and more obvious. They might form a dry crust and may begin to flake or shed. DO NO PICK OR PEEL. Do not wax or tweeze during treatments For best results multiple treatments maybe necessary. Its recommended that the first few treatments are no longer than four weeks apart. Avoid exposure to heat. This includes time spent in the sun, bathing/showering, cooking, saunas etc. Do NOT get laser hair removal done if you've started taking an antibiotic-regimen. Wait at least 2 weeks until after your are done with your regimen. Do NOT wax, use tweezers, nor thread the area in between treatments. When you notice hair regrowth, shave the area instead. If you have sensitive skin, rub ice continuously on the area which will numb it down before or after the procedure. Some hair may never completely go away. Stubborn-hair areas may need "touchups" once or twice a year. You will pay less per session if you buy a "pack" of session (usually about 6). If sensitive ask your doctor to suggest a local anaesthetic cream to apply before the procedure so that after and during the procedure there is less pain (Taking 2 Tylenol about 30 minutes before the treatment can be helpful too). Depending on the area being done, and the number of past session, the time between sessions may be between 4-6 weeks. 4. What recommendations would you reinforce to this client in view of the fact that she is going away on holiday tomorrow? You can NOT be treated if you have had ultra violet (UV) exposure from the sun or sun beds within the past 4 to 6 weeks. Therefore during your holiday you must cover well with clothing and hats and sun block to prevent extra activity of the melanin cells in your skin. Melanin cells are responsible for tanning. Your tan slowly wears off as those cells reach the surface of your skin in the normal skin- shedding activity, known as skin renewal.

Fake tans stain the outer skin cells, and this colour will intensify the IPL heat at the surface of the skin, therefore we cannot treat with IPL for at least two weeks after a fake tan application. I recommend shaving during your holiday so your hairs will be ready after you come back. SHAVING DOES NOT CAUSE FUTURE HAIR GROWTH TO GROW MORE RAPIDLY OR TO BECOME COARSER. Only the actual shaved hairs will become coarser, but they will shed, and new growth will be normal. Make sure you Hydrate the body by drinking plenty of water

5. What expected side-effects would you expect to see on his skin after a treatment. In general, laser procedures have a low risk of adverse side effects. The following potential temporary side effects are considered normal for the first few days: Itching Erythema (redness) Swelling around mouth of follicle (bumps) Tingling or feeling of numbness Subsequent to treatments you may experience some redness and/or warmth at the treatment site, which is normal and can be cooled with cold wet compresses if desired. It is recommended to avoid vigorous exercise, perfumed deodorants, heavily chlorinated pools and / or hot baths, saunas, etc. for 12 24 hours subsequent to treatments. For a day or two afterward, the treated area of your skin will look and feel like its sunburned. Cool compresses and moisturizers may help. Over the next month, your treated hair will fall out. Wear sunscreen for the following month to help prevent temporary changes in the colour of the treated skin. Blisters are rare but are more likely in people with darker complexions. Other potential side effects are swelling, redness, and scarring. Permanent scarring or changes in skin colour are rare. Some clients experience hyperpigmentation changes in the skin colour. If this occurs, the skin will typically return to normal pigmentation over time, however in rare cases may persist permanently. All treatment results are individual. If you expose your unprotected skin to UV light before and after treatments, you are seriously increasing your chance of adverse side effects, including burns and discolouration, so avoid unprotected sun exposure for 4-6 weeks before and after with an SPF 30+ sunblock.

6. Note the treatment parameters you would use for the patch test. Use the manufacturers instructions for the IPL or laser device that you use in the Australasian College Clinic.
