How to use this Risk Assessment Form
1. Detail the locations (actual workplace) of the hazard, risk or issue.
2. Describe the hazards, risks or issues.
3. Print your name and any names persons participatin in the assessment.

#. Determine the $ikelihood that loss, damae or in%ury would occur as a result of the hazard or risk, usin the &isk 'alculator criteria for likelihood.

*. Determine the &isk +core by usin the &isk 'alculator and your $ikelihood and 'onse)uence scores. ,his will i-e you a risk score of ./treme (.),
0ih (0), 1oderate (1), or $ow ($). Place this in the column labelled Risk Score (Before Controls).
2. Determine the control measures to be implemented to control the hazards3risks, consider both short term (temporary) and lon term (permanent)
control options.
4. 'onduct a second risk assessment to determine the risk score (usin the &isk 'alculator and repeatin steps #5*) after the control measure is
identified and selected for implementation. ,his will enable you to determine if the risk score has been satisfactorily reduced. Place this risk score in
the column labelled Risk Score (After Controls).
16. 7llocate a responsible person to ensure the control measures are implemented and a proposed date for their completion. 8our +uper-isor must -erify
