I.
a. Family Profile

The family consists of 10 members including the head, his wife, and a granddaughter, only 7 out of his
10 children are living with him because they already found work outside Camp One, Tuba, Benguet. They own
their house, so as with the lot for already 25 years.

b. Source of Referral

They were referred by the Health Workers and the Neighborhood nearby.

c. Reason of Choice of Study

I would like to inform the body why we chose this study because of the following reasons:

1. The family belongs to a low income
2. The disease (hypertension) is hereditary and find out how it is also a lifestyle
disease.
3. How is the disease being cured
4. How the family accepts the presence of illness/disease.

II.
a. Family Structure and Characteristics of the Members of the Family

Members of the household of Dela Cruz family are Mr. Fernando he is the head of the family, 49 years
old and he is happily married with Mrs. Rosalina Dela Cruz, 44 years old. They have (10) ten children but only
(7) seven are living with them namely, Fernalyn, 25 years old, female, single and she is the eldest, fourth child
is Anthony, 20 years old, sixth child is Felisa, 18 years old, seventh is Fernando Jr., 15 years old, Ferdinand, 12
years old, Ferdo, 9 years old, and Mariel 7 years old. They are also with their one granddaughter Amparo Lyn, 3
years old. The place of their residence is at Camp One, Tuba, Benguet. The type of their family structure is
extended and patriarchal. In terms of decision making the dominant family member is the head of the family
Mr. Farnando Dela Cruz. The family relationship is they talk or communicate to each other respectfully, no
observable conflict between members. They respect and love each other especially their children.

b. Socio-Economic and Cultural Factors

The source of monthy income of the family is Mr. Fernando Dela Cruz the head of the family, he is
working as a blacksmith. His income is exact for the basic necessities like foods and clothing, sometimes they
cannot even buy some extra clothing because of insufficient income for his family, not intended for
hospitalization or for regular check-up. The income of the family ranges from P200 300.00 per day. He is the
one who make decisions on how the money is spent. The educational attainment of Mr. Fernando Dela Cruz is a
high school level, and his wife Rosalinda Dela Cruz is also a high school graduate and she is a housewife. Their
Children, Fernalyn, Anthony, and Feliza already graduated high school, while Fernando, Jr., Ferdinand, Ferdo
and Mariel are all still in Elementary level. They lived in Camp One for a long time but doesnt have their own
house until they were able to buy the lot his cousin was offering him. The family is not aware of any activities
or projects in their community. They are not involved in its activities, and not a member of any organizations.
But the family is aware that there are existing organizations in the community.

c. Environmental Factors

As what I have observed and gathered information, there are different environmental factors that could
affect the health of the family.

According to the information given by the family, it is their owned house and their own lot. Their house
is made up of wood. They said that the living space is adequate for them. They have their owned appliances like
radio as a source of news and also their diversional activities, television and iron. There are presence of
breeding or resting site of vectors of disease like mosquitoes and flies, and there is a presence of accident
hazards wherein they are at risk for falls and vehicular accidents. The type of their garbage disposal is dumping
burning. Their waste disposal is flush, but their drainage system is open. The source of their water supply is
from public deep well, they cover their drinking water storage wherein it is placed in the plastic bottles. For
their food storage they just cover it. As we observed there are common household pets found in their house like
dogs and cats. There are many resources that they can utilize as a health source like herbal medicines which are
available to their environment. They also have health centers in their barangay but it is far from their house.

III. Identified Health Problems

The Assessment Process

The available data gathered during the first-level assessment reflected the following possible health
conditions or problems

1. Hypertension
Supporting Cues: Fernando Dela Cruz, Sr., 49 years old, father of 10 with a BP of 140/100 and is having
difficulting of breathing and a little dizzy. Maintenance of Metoprolol given at the Health Center.

2. Cough and Colds
Supporting Cues: Ferdo, 9 years old, Grade 4 student has cough and colds started two weeks ago.

3. Urinary Tract Infection
Supporting Cues: Amparo Lyn, 3 years old, diagnosed with Urinary tract infection. Taking Ambroxol and
Cefixime.

4. Lack of Immunization
Supporting Cues: Mariel doesnt receive any immunization.

5. Inappropriate environmental sanitation
Supporting Cues: The family doesnt have a reliable water supply that they can drink. Presence of breeding areas for
insects was also inspected. The family is dumping their garbages just near their house and when its becoming crowded,
they are burning it.


The result of the in-depth second level of assessment on the identified health condition or problems are
presented in the following tables.

Mr. Dela Cruz verbalized I feel alright
with the previous months and when I feel
difficult in breathing and a little dizzy I
passed by at the hospital for check-up and
I was diagnosed of having hypertension

I am a blacksmith and I am too busy with
my work thats why I dont have enough
time to go to the clinic for regular check-
up.

The clinic or health center is 3km away
from their house.

Ferdo, 9 years old, weighs 25 kgs, he has
cough and colds

Mother verbalized Ferdo is having cough
and colds a week ago

Ferdo is a Grade 3 student, her eldest
sister is taking care of their children, and
he is left in their house with his mother.

Mother is very busy working in their
house and she cannot properly take care
and give proper attention to her children.
















Amparo Lyn have Urinary Tract Infection

Mother verbalized I have such a lot of
things to attend when Im working in our
house, and I couldnt even see or attend to
the health needs of my children due to
lack or insufficient financial needs. My
husband was able to buy a medicine for
them, but due to insufficient financial
resources he cannot buy another one when
it is already consumed.

Family income is ranged from P200
300.00 a day

Water supply is from a public deep well or

Mariel the youngest child did not yet
received any immunization

Mother verbalized I didnt bring my child
to the health center for immunization
because it is too far from our house. My

too many already, we are burning it, usually in
the morning.

washing clothes, and drinking.
