LESSON PLAN TEMPLATE

Fill in all of the following information. Email to the appropriate internship faculty for approval at least one
week PRIOR TO giving the presentation.

Title of Presentation: Make-A-MyPlate

Target Audience: (describe the group, including age, setting of the class, and any other pertinent information):

Date, Location and Time: 04/24/14, Aventura Hospital Suite 1002, from 12-1pm. The setting takes place
in a large room. There will be 50 children expected. These are children of hospital employees because it is
bring your child to work day. There will be other adults present in the classroom to assist with
maintaining control of the class as necessary.

1. Describe the MyPlate diagram and its components
2. Name the 5 major food groups
3. Give an example of a food from each food group

2. Ice Breaker: Have children form a large circle and toss ball. State your name and how old you are
when the ball is passed to you.

Make-A-Myplate
1. Break children up into 5
teams.
2. Each team receives a
blank large poster paper
and crayons/markers.
3. Each team must draw a
MyPlate and color it in
correctly.
4. After coloring the
MyPlate, children must
create a real life MyPlate
using the food models.
Food choices must be
healthy in order to count.

Hold up MyPlate poster
Ask By a show of hands, how
many of you have heard of
MyPlate before? How many food
groups are there? Can you name
them?
Answer MyPlate can be used as
a guide and reminder for us to
eat from each food group in
healthy proportions. Eating from
each food group will provide us
with the nutrients necessary to
keep our bodies healthy.
Engaging in physical activity for
at least 60 minutes everyday is
also important
Review MyPlate:
Fruits: Half of your plate should
be fruits and vegetables. Fruits
and vegetables provide numerous
nutrients that help the body stay
healthy. Eating a variety of fruits
and vegetables will provide you
with different nutrients.
Vegetables: There are 5 sub-
groups of vegetables: dark-green,
red & orange, beans and peas,
starchy, and other. It is best to
eat a variety from each subgroup
to get all the nutrients. Can
someone give me an example of
each?
Grains: There are two subgroups:
Whole Grains and Refined
Grains. It is recommended that
half the grains consumed should
be Whole Grains. This is because
Whole Grains contain more
nutrients than Refined Grains.
Protein: Eat lean or low fat meat,
chicken, turkey, and fish. Other
sources of protein include beans,
peas, nuts, seeds, and eggs. It is
recommended that we consume
at least 8 ounces of seafood a
week. Or, eat seafood at least
twice a week. Can someone
show me what 8oz looks like?
Dairy: Choose dairy products
(such as milk, cheese, or yogurt)
that are Low fat (1%) or fat free.
5. The first team to finish
making a MyPlate wins a
prize!
6. When the game is over,
each team will be assigned
as a food group.
7. Each team must state the
recommendation for that
food group.
8. If the team answers
correctly, then they win a
prize as well.
Dairy products contain nutrients,
such as calcium, that help build
strong bones.

4. Evaluation Method/ Activity: Pass out short survey after activity. Ask children to rip off bottom piece of
paper and submit in box at front of the room.
