CAD Dispatcher comments :07/08/2014 00:47:05 - Window to house busted out
Ofc. was dispatched to 168 Sycamore Street in reference to property damage. Upon arrival, ofc. contacted Jaclyn Morris. Ms. Morris stated that
sometime within the past two hours, an object had struck her front upstairs bedroom window and cracked the glass. Ms. Morris did not hear anything,
but her four year old daughter had told her she heard something hit the window. Ms. Morris had spoken to her neighbors and no one had seen anything
out of the ordinary. Ms. Morris stated she has no insurance.
Ofc. noted the bedroom window was cracked and appeared to have been struck by a small object from the outside. The object did not completely
penetrate the window. Ofc. was unable to find the object in the yard or any other evidence in the area.
Nothing further at this time.

CAD Dispatcher comments :07/08/2014 01:56:27 - Small silver 2 door car occupied x2
On the above date and time officer was dispatched to the area of 238 Madeira in ref to a Suspicious Vehicle. Officer arrived on scene and noticed a
small Silver Honda, OH FAH7841, with a male subject sitting in the driver seat of the vehicle. Officer noticed no one else around the vehicle at which
time officer made contact with the driver who was found to be Justin Wiley. Mr. Wiley advised he was parking waiting on a friend, who he dropped off
and ran down the alley. Mr. Wiley advised he did not know what his friend was doing but he was waiting on him to return. Mr. Wiley could only advise
his friend was name "Chester" and Mr. Wiley did not know his last name. Mr. Wiley advised he and "Chester" were coming from Seventh and Hickory
and he was heading back that way once "Chester" returned. Mr. Wiley advised he drove to this location to drop "Chester" off.
Officer then gathered Mr. Wiley information from his and also checked the plates on the vehicle at which time officer found the plates were Expired
since 02/2014 and Mr. Wiley was under several License Suspensions. Officer then had Mr. Wiley exit the vehicle at which time officer performed a Terry
Pat for weapons. Officer felt several items in the right cargo pocket of the shorts Mr. Wiley was wearing. Officer felt a cigarette pack, along with a cell
phone charge but officer could not tell what the other items were. Officer asked Mr. Wiley if officer could empty his pocket and he advised "Yes".
Officer then pulled out the cigarettes, cell phone charged and officer also located a clear baggie with Green Vegetation inside and a pack of Rolling Papers.
Mr. Wiley advised the green vegetation was Marijuana and he forgot he had it in his pocket. Officer then seized these items as evidence and locate no
other contraband on his person.
Officer then issued a traffic citation, 21862, for DUS, No OL, and Expired Plates as Mr. Wiley advised several times he drove the vehicle to where he
was located at. Mr. Wiley was also issued 2 summons, 5049 5050, for Poss of Marijuan and Poss of Paraphernalia. Mr. Wiley signed for all copies and
was advised he needed to appear in court on 07/18/14 at 0900 hours. Mr. Wiley advised he understood. The vehicle was towed by Hill's towing and Mr.
Wiley was released. Officer was unable to locate anyone else in the area and was unable to locate "Chester". Officer placed the Marijuana and Rolling
Papers into Evidence at the LEC for further testing. Officer has nothing further.

Ofc was dispatched to 1240 N Bridge St in reference to a possible scam of an elderly subject. Ofc contacted the bank manager who stated John Vasilik
is the subject she called about. Ofc was informed Mr Vasilik and a white female subject came into the bank. The female sat in the waiting area and Mr
Vasilik attempted to withdraw $500 from his savings. The manager informed Ofc Mr Vasilik's daughter has his account set up so that her signature is
required for any withdraws made by Mr Vasilik. When denied the money, Mr Vasilik turned to the female and informed her. Reportedly she replied,
"That's ok, I can wait until another time". Mr Vasilik and this female then left the business, stood outside for about 8 minutes, then both returned inside.
The female again sat in the waiting area and Mr Vasilik went to the counter, were he cashed a $605 check written to him by a third party. The bank
cashed this, since his account did not prohibit this. Mr Vasilik and the female then left the business, both entering his vehicle. Video shows Mr Vasilik
handing the bank envelope to the female. They sat there for about 5 minutes and a white Toyota Rav pulled up, the female exited Mr Vasilik's vehicle
and entered the Toyota, which then left the lot. Mr Vasilik shortly then returned inside and asked to deposit the bank envelope into his savings account.
The teller found the envelope to be only $105. When asked where the other $500 was, Mr Vasilik was unable to tell the teller, so police was called. Bank
management stated they were attempting to contact his daughter (power of attorney) to explain this incident to her. Management also stated they were
going to flag his account so this does not happen again. Ofc questioned Mr Vasilik about this incident. During this incident, Mr Vasilik's long term
memory was very sharp, but his short term memory appeared to be very fuzzy. It appeared to Ofc that Mr Vasilik wanted to give this female $500 for
some reason, but he was unable to relate this to Ofc. Ofc informed Mr Vasilik and the bank management that this was going to be a civil matter but this
would be documented.

Ofc was dispatched to LEC lobby ref to a possible intoxicated male subject. Ofc arrived and observed a w/m, later identified as Christopher Craft,
sitting on the bench in the front lobby and was not responding to any questions and was slurring his speech and falling over. Christopher then became
unresponsive and collapsed and fell over and his lips and face began to turn purple and he was not breathing. The RCJ nurse came out along with Sgt
Cunningham and the subject had a faint pulse and a squad was summoned. Ofc's gave the subject multiple sternum rubs and his color began to come
back, but he was still unresponsive. The CFD medics arrived and administered a nasal dose of medication to counteract the obvious drug overdose and
they transported the subject to the ARMC emergency room for further treatment. Ofc learned that Christopher Craft was on parole under the
supervision of APA Amanda Sutherland and resided at the EXIT Program for offenders at 377.5 E 2nd Street and Ofc contacted the APA office and
advised them of the incident and that Mr Craft was transported to ARMC. Nothing further.

Ofc was dispatched to 81 S Rose St in reference to a breaking and entering complaint of a vacant residence. Ofc contacted Mark Cenci (Realtor) who
stated this house has been vacant for some time and is currently owned by Kingston National Bank (who is self-insured). Mr Cenci stated almost all of
the plumbing fixtures and piping was stolen. Mr Cenci stated a window on the north side of the residence was found open and he believes this was the
point of entry. Mr Cenci is unable to provide a time frame for when this could have occurred. Ofc informed Mr Cenci this report would be completed to

Robert Pack reported that he allowed Joseph Strausbaugh to borrow his bike eight days ago and he had not returned it. Pack described his bike as and
older three speed Huffy with a front hanging basket and a small rear red light.
Officer contacted Joseph in the area of Dairy Queen on Bridge Street. He was asked about the bike which he stated he was arrested for criminal trespass
several days ago and he told the Officer about Pack's bike. He advised the bike was against the Dollar General building on East Main Street.
Officer found Joseph was arrested by Officer Short, report number 14-11905, for trespass from Speedway East Main Street.
Officer searched both Dollar General and Speedway and could not find the bike. Officer also check the found bike area at the Law Enforcement Complex
and it was not there.
Officer also spoke with Officer Short who did not remember Joseph say anything about a bike.
At this time Mr. Pack's Huffy bicycle is lost or stolen and Officer was unable to find it.

SUBJECT IS IN THE LOBBY OF THE LEC
Ofc. was dispatched to LEC in reference to theft complaint. Ofc. arrived and spoke with Ruth L. Shrewsbury, who is the complainant. Ruth advised that
sometime on June 19, 2014 she had lost her Capital One credit card. Ruth explained that she realized she had lost her card when she received a call from
Capital One advised that her credit had been used and been denied twice on June 20, 2014. Ruth told Ofc. that someone had tried to use the card for
$229.00 to buy shoes from Eastbay and tried again to use it for $23.95 to pay some type of phone bill. Ruth canceled her card and wasn't out any
money, but needed to file a report about her card being used. Ruth had no information about who had tried to use her card due to the purchases being
used over the internet. Ruth advised that she believes that she may have lost her card at Mary Lou Patton Park located at 339 Douglas Ave. Ofc. has no
suspects at this time.

Upon arrival Ofc contacted Menards LPO April Shoemaker, whom stated she witnessed (on surveillance video) Andy Munyon select a Real Tree brand
pocket knife from the merchandise shelf then removed the knife from the package by cutting open the plastic packaging material with another pocket
knife. April stated Andy then concealed the Real Tree brand pocket knife in the pocket of his shorts and exited the store passing all points of sale
without ever attempting to purchase the concealed item.
April stated she contacted Andy in the Menards parking lot where she identified herself as LP and escorted Andy back to the LP office where the stolen
item was surrendered.
Ofc contacted Andy and informed him of his Miranda Rights which he understood and waived. Andy then provided the same account as April by
admitting to the Theft. Andy stated he attempted to steal the item because his pocket knife was broken. Andy was placed under arrest and transported
to RCJ where he was charged with Theft with Court Date: 07/09/2014 at 9am.
The pocket knife was photographed and returned to LP in exchange for a sales receipt. LP will provide further evidence when its available. Nothing
further.

Brent Butterbaugh reported that his Samsung Galaxy S5 was stolen from a vehicle. Brent stated that he caught a ride with some of his family
members to Sack n Save. Brent stated that they picked up two other people, with who he is not familiar, on the way. Brent stated that after they all
arrived at Sack n Save his phone came up missing. Brent stated he did not see anyone exit the vehicle with his phone nor did he see the phone in the
possession of anyone else. Brent stated he knew the phone was in the car and believes one of the two strangers took the phone. Brent did not have the
serial number for the phone but does have insurance through Verizon on the phone. Brent stated that everyone in the car denied having any knowledge as
to where his phone might be.
Ofc has no suspects or leads at this time.

Officer was dispatched to Kohl's in reference to a theft who left in a white chevy car. Upon arrival Officer saw loss prevention, Phil Coen walking across
the parking lot. He advised he observed a white male in the fragrance department select a bottle of $69 men's cologne and conceal it in his left front
pocket. He advised the white male exited the store failing to pay for the merchandise at which time he was stopped past all points of sale out side the
business. Coen advised the suspect handed him the bottle of cologne to the white chevy, Ohio license plate number FTJ9145, which comes back to Beth
Whalen of 400 McKellar Street Chillicothe on a white 2006 Chevy.
Officer called Ms. Whalen who stated she allowed her daughter Megan Whalen to borrow her vehicle today. Officer gave her the description of the male
which was also found in a picture on her Facebook page. Ms. Whalen stated that would be Taylor Hicks who is dating her daughter. Ms. Whalen gave
Officer his phone number.
Officer called Taylor on the phone and he was asked to met Officer at the Law Enforcement Complex. Officer informed him it was about him being at
Kohl's and he stated he was and forgot to pay for bottle of cologne and when he gave the bottle back got scared and took off running to the car. He
advised he was sorry and would met Officer.
Taylor did not met Officer today as he said he would. Officer attempted to contact him and the phone has been disconnected. Officer will attempted to
call him tomorrow, July 9, 2014 and if he is not contacted and meets officer a warrant will then be requested for theft.

OLDER MODEL GRAY COLORED OLDSMOBILE
Ofc responded to the parking lot of 99 S Bowman Rd in reference to (2) subjects whom appeared to be under the influence of drugs and were now sitting
in a gray colored, older model Oldsmobile 4dr with Indiana registration.
As Ofc was enroute, Ofc was informed by dispatch that CFD was now on scene with the (2) subjects. Ofc arrived and made contact with CFD whom
stated as they were traveling s/b on S Bowman Rd when they observed John Davis (driver seat) and Keystan Davis (passenger seat) occupying a gray
colored, older model Oldsmobile 4dr parked in the parking lot of 99 S Bowman Rd. CFD informed Ofc that both subjects appeared to be passed out in
the vehicle as they both had there heads slumped over their shoulders. CFD stated they made contact with the occupants but had to shout loudly for the
subjects to respond. CFD stated both subjects had pin-point pupils and appeared to be very lethargic and possibly under the influence of drugs.
As Ofc contacted the occupants, Ofc did note the keys were not located in the ignition of the vehicle and the vehicle was not running. Ofc first made
contact with John Davis whom stated he and his wife, Keystan Davis, were at 99 S bowman Rd to visit a friend. John stated his friend (that he
identified as Josh Smith) was not at his residence upon their arrival. John stated he and Keystan returned to their car and chose to wait in their vehicle
until Josh returned to his residence.
While speaking to John, Ofc noted he had glassy, pin-point pupils, slightly slurred speech, and appeared very lethargic. John denied taking or using any
drugs other than his prescribed dose of Methadone on this date. John informed Ofc that there were no other drugs, weapons, or contraband in his vehicle
(1986 Oldsmobile Delta 88 IN-WFG361) other than his prescribed medication. John then gave Ofc consent to search his vehicle and his person for
drugs, weapons, and contraband. Ofc did locate (1) bottle of Methadone tablets (white colored round tablets inscribed 54/142) in John's right front pants
pocket that was prescribed to John on 07/07/2014.
Ofc then contacted Keystan Davis whom provided the same account as John. Keystan also stated she has only taken her prescribed medication on this
date and denied using any other drugs or medications. However, Keystan did state she consumed (2) prescribed Ambien tablets instead of her prescribed
dose of (1) tablet. Keystan also appeared extremely lethargic, had glassy pin-pointed pupils, and extremely slurred speech. CFD offered to treat
Keystan on scene but she refused. Keystan was able to answer all questions asked by CFD regarding time, place, and location.
Keystan then gave Ofc consent to search Keystan and her belongings in the vehicle for drugs, weapons, and contraband. While searching Keystan's
purse, Ofc located a bottle of Ambien medication prescribed to Keystan that also contained (10) white colored tablets inscribed 54/142 that Keystan
identified as Methadone tablets belonging to John. Ofc informed Keystan of her Miranda Rights which she understood and waived. Keystan stated the
medication was given to her by John to store in her prescription bottle. Keystan denied possessing or using the Methadone tablets.
Ofc then discovered through LEADS that Keystan had an active warrant from this agency for probation violation. The warrant was confirmed and
Keystan was placed under arrest for the warrant. While in custody in the rear of Cruiser 19, Keystan informed Ofc that she believes she has overdosed
and requested CFD to return to the scene for treatment. Keystan informed CFD and Ofc that she has used heroin and orally consumed (60) Oxycodone
tablets on this date. Keystan then informed Ofc that their is heroin located in a hidden compartment within the console of John's vehicle.
Ofc searched the vehicle and located a bottle of Oxycodone tablets (prescribed to Keystan Davis on 07/07/2014 by Dr Patrick Sheets) within the fabric
of the center console of John's vehicle. Ofc noted the prescription was for (60) Oxycodone tablets but the bottle only contained a $1 bill and (3) brown
colored substances wrapped in white paper that Keystan identified as Heroin belonging to her and John.
Ofc contacted John and informed him of his Miranda Rights which he understood and waived. Ofc informed John of the discovery but he denied
possession of the substance. John also denied knowing the substance was located within the vehicle and stated the vehicle belongs to a friend from
Indiana. John stated he and Keystan traveled to Ohio for a custody hearing that Keystan was required to attend.
John consented to a series of SFST's which he failed to perform accurately. While performing the HGN, Ofc noted John did not have any presence of
Nystagmus, but he was unable to focus on the stylus and could barely keep his eyes open. John also failed to complete the walk and turn test and the
one leg stand test as directed. For the safety of John, he was placed under arrest and transported to RCJ where he was issued MM citation #5102
charging Disorderly Conduct Intoxication with Court Date: 07/14/14 at 9am.
Keystan was transported to ARMC-ED where she received treatment for possible overdose. Ofc photographed/seized the bottles of medication
prescribed to Keystan containing the suspected Heroin and the suspected Methadone. Ofc will submit all evidence to BCI for further analysis.
Vest towing arrived and transported the vehicle to their impound lot. A tow sheet was completed and Ofc will attempt to contact the vehicle owner via
correspondence.
Investigaiton continues pending lab results from BCI.

Officer was dispatched to 324 East Main Street in reference to a domestic dispute that happened at 184 East 7th Street. Upon arrival Officer spoke with
Gail Keaton who stated she and Jason Lytle had been dating and also have a young child together. She advised today she went to break up with him and
get her key back to her house and they got into an argument. She claimed he pushed her and she got back up and spit in his face. She advised they then
got in each others face and she pulled her hand back to hit him but did not. She advised they argued more and she left with out her key. Gail advised she
only wants her key back and for Jason to be barred from her house. She did not want any other action taken.
Officer saw no sign of injury on Gail.
Officer then contacted Jason at 184 East 7th Street. He did not admit to pushing Gail down but did say she spit in his face and they argued. Jason gave
Officer her key and he was barred from her house which he stated he understood. Officer also did not see any injuries on Jason.
Officer returned the key to Gail.
Officer is only able to prove that both Gail and Jason argued and no other witness was around.

Ofc. was dispatched to the area of Western/University in reference to a possible fight involving a brown SUV. Ofc. arrived in the area and made contact
with Dominique A. Liggins, who was one of the parties involved. Dominique advised that he had been traveling in a vehicle with his girlfriend, Lindsey
N. Miller and they were arguing. Dominique explained that Lindsey swung at him, but missed so got out of the car to leave the situation. Dominique told
Ofc. that he was just released from prison on June 12, 2014 and he didn't want to be in trouble so he left. Lindsey was no longer in the area. Dominique
told Ofc. he didn't wish to pursue anything and that he would contact Lindsey after they have both had time to calm down. Ofc. has nothing further.

Ofc responded to 257 Annis Ct in reference to a domestic violence complaint.
Upon arrival Ofc contacted Linda Overly whom stated she and her husband, Thomas Overly, were involved in a verbal altercation over her children.
Linda stated Thomas was upset that she was fixing dinner for her daughter because he claimed Linda's daughter was able to care for herself. Linda
explained she believed Thomas does not like her children which causes frequent disputes and altercations.
Linda stated the verbal altercation continued as she was sitting on the back porch eating her dinner. Linda stated the altercation escalated from shouting
at each other to both subjects pushing each other. Linda stated during the physical altercation, Thomas picked up a fork from the table and attempted to
stab her in the chest with it. Linda stated she only sustained an abrasion to the chest from the fork which Ofc observed and photographed. Linda did not
report any other injuries or physical violence. Linda stated after she was injured from the incident, she immediately left the area and traveled to her
daughter's residence on Annis Ct. Linda completed a written statement documenting her claim. Linda refused medical treatment. Linda stated only she
and Thomas were at the residence during the incident and no witnesses were available.
Ofc arrived at Thomas and Linda's residence (450 Chestnut St) where Ofc discovered the front door to be open. Ofc shouted numerous times inside the
residence identifying this Ofc and requested Thomas to come to the door. After several minutes of shouting for Thomas, Ofc entered the residence and
located Thomas in his upstairs bedroom asleep on his bed. Ofc once again had to shout at Thomas and tap his shoulder for Thomas to respond. Thomas
had a very strong odor of an intoxicating beverage on or about his person, had glassy blood-shot eyes, extremely slurred speech, and was unsteady on his
feet. Ofc informed Thomas of the complaint and informed him of his Miranda rights which he understood and initially waived. Thomas then stated that
he and Linda were involved in a verbal altercation and that he picked up a fork and used it against Linda. Thomas claimed that he did not stab Linda with
the utensil but "poked" her with it. Ofc then asked Thomas if he made contact with Linda's skin and caused the injury that Ofc observed, but Thomas
refused to answer any further questions and exercised his Miranda Rights.
Thomas was then placed under arrest and transported to RCJ where he was charged with (1) count of Domestic Violence (M1). Thomas has no prior
Domestic Violence offenses as confirmed with LEADS CCH. Thomas will remain incarcerated until Court Date: 07/09/2014.
A Domestic violence packet was provided to Linda and she was informed of how to obtain a TPO.
