TESTIMONY

Lucy Lopez, cashier at Mainstream Business


Employed since 2003
I used to work in their Makati Branch but when they opened a store in Baguio
City, I asked to be transferred there.
Since then, I have been working at the jewelry section of Mainstream Business
Department Store in Baguio City, also as a cashier.
I am diligent and good at what I do. I even got awarded as Employee of the
Year last 2012. I was also never the subject of any disciplinary action.
On duty on August 15, 2013 from 10am to 7pm
It was a busy day at the mall, in general. Madami pong customers. Some were
just checking out our goods but I can say there were a good number of
purchases that day mas marami po kesa sa usual na dami nang nabibili.
From what I can recall, sometime in the afternoon, a woman came in and alleged
that a transaction was made using her allegedly stolen credit card.
She asked for the copy of the signed receipts of two certain purchases made
within that day. So pinakita ko po sa kanya pero sabi niya hindi daw po yoon
yung pirma niya.
Una, syempre po nagtaka po ako, sa totoo lang. Sa sampung taon po na kahera
ako, at sa dami nang customer na gumamit ng credit card, kahit kailan wala pa
pong ganoong insidente.
POLICY:
o Lagay items sa cashier
o Bigay po yung credit card
o Hingi po ako ng valid id
o Kapag binigay po yung credit card, inacknowledge ko po yung card.
Sabihin kop o yung pangalan. Tapos mag-yes po yung customer
o Tapos bago kop o iprocess, ioffer ko po yung promo, kunwari po 3 mos
installment
o Tapos ibibigay kop o yung copy sa customer para poi sign
o Check ko po if signatures sa credit card and sa resibo ay tama
o I check if the three signatures yung sa credit card, sa pirma sa resibo,
and sa valid ID are the same.
o Syempre po we also check if kamukha nung nasa valid ID yung customer
na nag-present ng credit card.
I recognize her face. Siya po yung nabanggit kong customer na nagreklamo.
Naaalala ko po kasi yoon lang po yung only time na may ganoong incident.

Extra:
Did you confront the sales personnel concerned at the counter/booth with what you
said you found out and your claim of unauthorized use of your credit card as you just
stated?
Yes, sir, but a lady supervisor thereat advised me to present my complaint to
their Customer Service Office and instructed a lady personnel to accompany me
to said office.

What happened at the Customer Service office?
The personnel there called thru intercom and ordered that the charge
invoices/receipts be brought to the customer service. When the charge
invoices/receipts were brought in, the personnel showed them to me and I told
him the signatures in the charge invoice/receipts are not mine and differ from
my signature in my stolen card.
