A. Considerations. Melanie Fuller, of Oakland Tn, OR, (hereinafter referred to as adopter/buyer), agrees
to sell, and ________________ ___________________ (hereinafter referred to as buyer) agrees to
adopt one standard poodle, AKC litter registration number __________________
born ________\ _____\ 20_____ (herein after referred to as dog.)

B. Delivery. The dog is to be delivered to buyer on _______________________. The buyer is
responsible for all shipping charges both to and from the seller.

C. Limited health guarantee. Seller agrees to accept the return of puppy with no refund to adopter if
within 72 hours the dog is found to be suffering from infectious disease, attributable to the seller, as
determined by a licensed veterinarian. Guarantee will exclude internal and external parasites. Discovery
of disease must be documented in writing from the veterinarian and presented to the seller.
Replacement animals are to be selected from litter-mates or the next available litter, at the seller's
option.

D. Genetic health guarantee. In addition, if the dog is diagnosed as having hip dysplasia, sebaceous
adenitis, von Willebrands disease, or genetic eye disease prior to 12 months of age, seller shall at
buyer's option, replace dog under terms specified above (buyer pays for transportation but not a new
dog). Written proof of spaying or neutering of afflicted animal is required before replacement animal is
provided. Seller may require OFA (Orthopedic Foundation for Animals) confirmation of hip dysplasia,
CERF confirmation of eye problems, and authoritative confirmation of sebaceous adenitis, as well as

documentation from veterinarian involved.

E. Buyers obligations for responsible pet ownership
Buyer agrees:
1. That this Dog is being adopted as a family member and will be given extensive house privileges;
i.e. sleep inside the house, be allowed freedom of the house when the family is home, and will NOT
be restricted to the yard at any time. You agree to provide the Dog with fresh water, and a high
quality grain free dog/puppy food, adequate outdoor exercise, affection and shelter. You also agree
that the Dog will not be used as a guard animal, or for any purpose other than as a companion and
friend to you and your family.
2. That this dog will carry the seller's kennel prefix, Super Nova , and will consistently be exhibited in
matches and any AKC sanctioned events under such registered name, and,

3. If at any time in the future you cannot continue to provide proper care for the Dog, you will not
abandon, give or sell the Dog to another person, company, organization, medical research, pound or
animal shelter. You agree to do one or more of the following:
A. Obtain approval from Super Nova Standard Poodles to transfer the ownership of the Dog to a
friend or relative. Rescue must approve the new adopter who will be required to sign an agreement
similar to this one.
B. Agree to cover the costs to board Adoptee for an agreed-upon time, allowing Super Nova
Standard Poodles to actively seek a new home and then relinquish the Dog to us when a home is
found. Note: Boarding costs are $25 to $30 per day.
C. Return Adoptee to Super Nova Standard Poodles in good health and condition at adoptees expense.

4. The Dog will be examined by your licensed veterinarian within 72 hours of adoption and will be
provided with medical treatment as needed. You agree to keep all vaccinations current including but
not limited to Rabies, Distemper/Parvo (DHLPP), Bordetella, AND to ensure the Dog receives
preventative medication to protect against heartworm if necessary. You agree to provide the best
available care to the Dog in a timely manner. Any veterinary problems that may arise, including
serious illness requiring emergency care or surgery is your responsibility and you are responsible for
all related costs.

5. To have this dog groomed on a regular basis as required for good health and appearance, and,

6. That neither this dog nor its offspring will be offered for sale to pet shops, puppy mills, or their
representatives or suppliers.

7. You agree that the Dog will ALWAYS be wearing an identification tag with two current telephone
numbers plus the microchip tag that was provided.


8. Where applicable, adopter will provide a secure fenced yard, with ample room for outdoor
exercise. If fencing needs repair or modification in order to ensure the safety and well-being of the
Dog, you agree to make any necessary repairs prior to allowing Adoptee access to the yard. You
agree to maintain the security of the fencing in the future to further secure the Dog. Adopter

understands that gardeners, pool men, contractors and utility workers are not responsible for the
safety of the Dog, therefore adoptee must be kept indoors when workmen have access to the yard.
NOTE: Many dogs are sensitive and scared of loud noises, and you hereby agree to keep the adoptee
indoors or in a safe and secure environment on holidays such as July 4
th
and New years

9. If the Dog exhibits any behavioral issues (i.e. biting, destruction, escapism, fearfulness,
dominance) that might lead to abandonment of the animal: Adopter agrees to contact a trained
professional trainer to help resolve the behavior issues and to follow the recommendation in
resolving any issues as an effort to maintain custody of the adopted animal. If there is no resolution
you agree to NEVER surrender the dog to another person, company, organization, medical research,
pound or animal shelter. That you will contact Super Nova Standard Poodles for further evaluation,
and return the dog to us if necessary.

10. You will walk the dog on a leash when on public streets or in parks or other unfenced areas, and
will never allow the Dog to roam free unsupervised. The Dog will be walked using a slip or choke
collar, pinch collar or harness, and will not be walked with the leash attached to the Dogs
identification tag. Adopter understands that the identification tag is not a secure restraint. Also, the
Dog will NEVER be tied up on your property or elsewhere. Adopter understands that restraining a
dog by tying it to a fixed post or wire run is unsafe and may cause theft of the Dog, unpredictable
behavior and random aggression. You will not leave the dog crated and/or otherwise confined for
extended periods of time (more than 5 hours)

11. If the dog becomes lost you agree to contact Super Nova Standard Poodles immediately so we
can aide in the search of the dog and then the adoptee is to make an immediate and serious effort to
find the dog, by (a) filing lost reports with the local police, animal control authorities, animal
shelters, SPCAs and local veterinarians and pet stores, (b) post lost dog signs (c) place lost dog ads in
your local paper (d) contact us for additional support and advice. Furthermore, Adopter understands
that if the loss was the result of negligence or failure to comply with the covenants of this
Agreement, Super Nova Standard Poodles reserves the right not to return the Dog and to place in
another home.

12. You agree that you may not euthanize the Dog except in the case of the Dogs terminal illness or
injury, or old age accompanied by pain and suffering and in that case, the euthanasia must be
performed by a licensed veterinarian. And Super Nova Standard Poodles must be notified within 24
hours of such case in writing, with the attached veterinarian documentation


13. You will notify Super Nova Standard Poodles of all changes of address and/or telephone within
30 days after the change. Communications should be mailed to: 110 Arborlon Cove , Oakland Tn ,
38060.

14. Adopter agrees to provide for care of the Dog in their will or estate if by no other means than
specifying in writing that Adoptee is to be returned to Super Nova Standard Poodle in the event of
Adopters death. Living family members must contact Super Nova Standard Poodles and notify then
of the passing of the adoptee and follow then they are to follow the No right to transfer agreement
initialed above and follow its conditions.

F. If this Dog has not been sterilized prior to this adoption due to age or physical condition, you agree
to enter into a Spay / neuter contract with Super Nova Standard Poodles to have the Dog spayed or
neutered in accordance with the spay / neuter contract and you agree to a by adhere to the conditions
of this contract or face the described consequences.
Furthermore, Buyer agrees that this dog is being purchased as a pet only, and that the guarantee of
show or breeding quality is neither made nor implied. Buyer agrees to have this dog altered (spayed or
neutered) at the age of six months. The registration form will be a co=ownership until the seller receives
a statement verifying the alteration from the veterinarian performing the surgery. Limited registration
papers are to be provided at that time.

G. Adopter understands and agrees that Super Nova Standard Poodles is fully entitled to make
follow-up visits or phone calls to ascertain that all of the covenants and clauses of this agreement are
being satisfied. Adopter is obligated to cooperate with us and comply.

H. Adopter understands and agrees that Super Nova Standard reserves the right to reclaim adoptee if
any of the above conditions are not to our satisfaction. Adopter further understands and agrees that
any expenses incurred by Super Nova Standard Poodles in exercising its rights under this provision
will be paid by Adopter in addition to any liquidated damages required by other provisions of this
Agreement.


Breach of Agreement/Liquidated Damages.

In the event that you do not comply with the terms of this Agreement, or the Dog is abused or
neglected, we hold the right to recover the Dog from you upon demand, and the Dog will be
surrendered to us immediately. In addition, without waiving any of our rights under this Agreement,
if you do not surrender the Dog to us, you will owe us a payment in the amount of $2,500 agreed
between us as just and reasonable liquidated damages to compensate us for our costs and expenses in
connection with your breach of this Agreement and we reserve our rights to reacquire the Dog.

Entire Agreement/Modification/Binding Effect:
This Agreement (including the attached Voluntary Release/Waiver of Liability) is the entire
agreement between you and us, and supersedes any prior understandings between you and us with
respect to the subject matter of this agreement. No modification of this Agreement will be valid
unless in writing duly signed by both you and us. This Agreement is binding upon you and your
heirs, assigns, successors, personal representatives and executors.

Additional conditions:

ONLY if the signature of Melanie Fuller , owner of Super Nova Standard Poodles, is represented here ,
x ___________________________________ does the adoptee of the above mentioned dog not enter
into a Spay / Neuter agreement . The adoptee understands that the dog will then be adopted and co-
owned with Super Nova Standard Poodles, until the dog receives its Championship in conformation or
agility. Once proof of completion is submitted in writing to Super Nova Standard Poodles, seller agrees
to sign over sole ownership to the adopter, at the adopters request and sole expense. Seller with endure
no financial hardship for transfer or ownership.
